Corrected on Site - Voluntary Discarded. KDA_5_20212A - --The hot water at the hand washing sink is only at 80.5F. According to the employee, they have not had a hot water heater for a week. Note: The facility was instructed to voluntary close the kitchen and not cook anything until a hot water heater is put in and brings the hot water to above 100 degrees F. KDA_5_20515B - --There is water leaking from the faucet at the 3 Vat Sink when the water is turned off. KDA_6_50111 - --There are 6 ceiling tiles in the back kitchen area with water damage and these tiles are directly above the food prep are and the fryers. KDA_6_501112 - --There are 4 dried rodent excreta underneath the cabinet below the public microwave. KDA_7_10211 - --Underneath the Register, there is an orange bucket with clear liquid with no label. According to Employee, the clear liquid is Chlorine Sanitizer. Corrected on Site - Labeled.
