- KDA_3_30211A1b - --Located in the make table, there was a container of raw shrimp stored over a container of cheese sauce. COS- moved shrimp to bottom shelf.
- KDA_3_30212 - --Located in the storage room, there were three unlabeled containers of white powder which the employee identified as cornstarch, flour, and sugar. COS- labeled. Located on a shelf under the make table, there was an unlabeled container of white powder which the employee identified as cornstarch. COS- labeled.
- KDA_3_30511A1 - --Located in the storage room, there was an uncovered container of breadcrumbs stored on a shelf. COS- covered.
- KDA_3_50116A2 - --The following temperatures were measured from foods located in the Aquafina reach in cooler (RIC)in the kitchen: cooked chicken 49F, raw chicken 61F, raw chicken 58F, egg rolls 54F,and cooked shrimp 45F. The ambient temperature was 38.5. Employee indicated that the food had been in the RIC since yesterday. COS- discarded all potentially hazardous food and educated employees proper cold holding temperatures and to no over fill the RIC.
- KDA_4_60111A - --Located on the dish storage rack, there were several metal and plastic containers that were soiled with dried food debris. COS- moved dirty containers to wash area. Located in the WIC, there was bucket of onions that was not clean. COS- employee moved onions to a clean container. Located on the make table, there was a wooden sushi roller covered in plastic wrap that was soiled with dried food debris. COS- discarded sushi roller.
- KDA_4_60111C - --Located in the kitchen area, there was a microwave and two toaster ovens that were soiled with an accumulation of dried food debris. There is an accumulation of dried food debris and grease on the vent hoods above the grill area in the kitchen. The shelves and fan guards of the WIC were soiled with an accumulation of dust and dried food debris.
- KDA_4_60211E4 - --The ice bin located in the storage room had an accumulation of mold inside the unit under the rim.
- KDA_5_20213 - --The person in charge said that the produce is washed in the directly plumbed three vat sink. An indirectly plumbed produce sink needs to be installed.
- KDA_6_20211A - --Located in the above the three vat sink in the kitchen area, there are lights that are unshielded.

- KDA_7_10211 - --Located under the three vat sink, there was an unlabeled spray bottle of pink liquid. Employee identified liquid as dishwashing soap. COS- labeled.
- KDA_7_20111B - --Located on a shelf above the dish rack with clean dishes, there was a bottle of first aid spray. COS- moved first aid spray.
