- KDA_3_50116A2 - --Individual packets of butter in the condiment trays on the tables had a temperature of 61 F. The PIC said they were put on the table this morning. COS, VD.
- KDA_4_10111A - --On the counter in the kitchen biscuits were stored in direct contact with a non-food grade Sterilite container. COS, put in food grade bag.
- KDA_4_20211A2 - --On the counter in a crock in the kitchen 2 plastic spoons were melted on the ends. COS, discarded.
- KDA_4_60111A - --The meat slicer, stored as clean, has dried food debris on the back of the blade. COS, moved to be cleaned and sanitized.
- KDA_4_60111C - --There is dried food debris on the base of the meat slicer.
- KDA_4_90311A - --In the storage room boxes of single service cups, dome lids, and food storage containers were stored directly on the floor.
- KDA_4_90411B - --In the kitchen in the utensil holder knives, forks, and spoons are stored with the lip contact surface up.
