KDA_3_30412E - --In the back storage area there was one metal ice scoop being stored directly on a metal counter that had dust debris build up (not a clean sanitized surface). There was also a red plastic ice scoop being stored directly on the left side ledge of the ice machine with dust debris build up (not a clean sanitized surface). COS-placed the red plastic ice scoop in an ice bag.
KDA_3_30511A1 - --In the back storage area there was one box of Chex Snack Mix being stored directly on the floor.
KDA_3_50114A1 - --In the walk in cooler there was a plastic food grade container of gravy at 90.5F. It was placed in the cooler around 10:30am and was tempted around 12:50pm (past 2 hours). The container was sealed tightly and condensation was on the inside of the container and lid. COS-discarded. The ambient air temperature of the walk in cooler was reading 36.1F.
KDA_3_50118A1 - --On a retail shelf there were 3 loafs of Rotella's Italian Bakery Vienna bread with a best buy use date of 10/24/15. COS-pulled from the shelf.
KDA_4_60111C - --There is minor dust build up on the fan blowers inside of the walk in cooler. There is ice build up on the walls and ceiling of the walk in freezer.
KDA_4_60211E4 - --The is mold build up inside of the pop nozzles to the Pepsi soda fountain machine.
KDA_4_90311A - --In the back storage area there was one box of 32 oz plastic cups, one box of hot or cold insulated food containers (6 oz) and one box of 20 oz foam cups being stored directly on the floor.
KDA_5_20515B - --At the mop sink there was a shut off valve below the atmospheric vacuum breaker. The water was turned on and the shut off valve was turned in the off position (water can't flow freely-under pressure). One hose was connected to the chemical dispenser which is installed with a backflow preventive device and the other hose was hanging below the flood rim. The atmospheric vacuum breaker does not leak when water is supplied. KDA 61 educated about the constant pressure from the shut off valve on the atmospheric vacuum breaker and discussed placing a Vented T-Valve. COS-hung hose up to provide an air gap.
KDA_6_20215A3 - --There is a 1/4" gap at the bottom of the back exit door. This could allow the entrance for pests.
KDA_6_30114 - --There is no handwashing signs at the designated handsinks. COS-left handwashing stickers.
KDA_6_50112A - --There is trash on the floor in the back storage area and in the walk in cooler.
KDA_7_10211 - --On the counter there was a working spray bottle with blue liquid and not labeled.The person in charge said it was window cleaner. COS-labeled bottle.
