There was an unlabeled container of thick creamy substance held in the walk in cooler for an unknown amount of days. The employees believed it to be cream of broccoli soup. There was an unlabeled container of what appeared to be sliced potatoes held in the walk in cooler with the plastic wrap on the container bulged, a foam scum on the top of the container, and a very foul smell from the container. The last cooking event in the kitchen was 12/20/15, as per employee. "Corrected on site" (COS-foods were discarded).
KDA_4_20211A2 - --There was an orange colored food debris in an open seam of a rubber spatula hanging over the west prep table. (COS-discarded).
KDA_4_60111A - --There was dried light and dark colored food debris all around the guard of the stand mixer. The mixer was lasted used 12/20/15, as per employee.
KDA_6_501111A - --There were approximately four fresh rodent droppings on the north basement shelf under a case of chafing fuel. There were approximately two dozen fresh rodent droppings under the east side of the north basement storage shelf. There were approximately two dozen fresh rodent droppings under the south storage shelf in the basement. There were approximately eight fresh rodent droppings between roaster pans on the south storage shelf in the basement. There were approximately twenty fresh rodent droppings on the middle shelf behind a case of gravy boats and plates on the south storage shelf in the basement. There were approximately a dozen fresh rodent droppings behind a case of salt and peppers shakers on the south storage shelf in the basement. The facility monitors for pest. The facility treats for pest with glue traps which were located in the basement. The facility also has a pest control company treat on a quarterly basis, as per employee. The last treatment was on 1/20/16, as per invoice. There treatment was for spiders, crickets, and cockroaches.
