- KDA_3_20215 - --On 'clearance' shelf, a can of Hunts Tomato Sauce and Bushs Chili Beans had significant dents along hermatic seals. Corrected on site (COS) - cans thrown in trash.
- KDA_3_50116A1 - --In hot display case, RTE breakfast pizza was 112F. Manager stated it had been in warmer for approximately 3 hours. Establishment does not use TPHC for hot items. COS - pizza disposed of in trash (managers decision).
- KDA_3_50118A1 - --In small walk in cooler, an opened package of RTE corn dogs had consume by date of 12/16, 2 opened containers of RTE sliced deli ham had consume by dates of 12/17 and 12/14, and a container of RTE rib patties had consume by date of 12/14. All held over 7 days. COS - corn dogs, ham and rib patties were disposed of in trash.
- KDA_4_50111A - --The counter top between hand sink and 3-vat sink is worn down with the raw particle board exposed. The counter top on west side of 3 vat sink has top covering chipped, exposing raw particle board.
- KDA_4_60111C - --There is a build up of dust on all of the equipment in food prep area. There is a build up of lint on the vent hood filters above grill and fryer. Manager said they were cleaned weekly. There is a build up of lint on the fan guards in both walk in coolers. In small walk in cooler, the plastic shelf has a build up of mold on shelves. In display walk in cooler, there is food debris along the edges of cooler.
- KDA_6_20215A3 - --The back door has a 1/2 inch gap along the bottom right side of the door from the handle down.
- KDA_6_50111 - --There are 5 ceiling tiles with water damage/mold growth.
- KDA_6_501111A - --There are fresh rodent droppings, too numerous to count, under food prep counter, 3 vat sink, back storage room, and behind coolers. Manager stated they are serviced regularly by a pest control company. There were several tamper-resistant rodent bait stations in establishment.
- KDA_6_50112A - --There are fresh rodent droppings (too numerous to count) in cabinets of food prep area and along floor edges of back storage room.
- KDA_Defacto - --3-vat sink sanitizer level was 400ppm quat. --Cold Holding: Container with ice - sliced tomatoes 39F. Small walk in cooler - 39.6F ambient. Display walk in cooler - milk 40.4F. Gatorade cooler - 39.8F ambient. Pepsi cooler - 40.8F ambient. 7UP cooler - 40.3F ambient. Display cooler - cut lettuce 41.9F, cream cheese 38.8F, chopped deli ham 38.8F. Freezers ok. --Cooking temperature: Grill - hamburger patty (cooked from raw) 174F. Fryer - RTE burrito - 160F.
