- KDA_3_50118A3 - --There were 14 opened packages of ready to eat lunch meats (roast beef, ham, and turkey) in the Deli Reach in Cooler that were dated with one too many days. Note: Explained to the Deli employee that the date the original package was opened is day 1. In the Reach in Cooler (near cashiers) there were seven packages of ready to eat (prepared on site) beef sticks that were dated 11/19/15 and 10/30/15. Note: The manager said that these were all prepared 10/22/15.
- KDA_3_60211B3 - --There were no net weights on packages of "day old" donuts (offered in front of the donuts display).
- KDA_4_20211A2 - --The plastic brace on the arm of the Meat Dept slicer is worn with cracks and frays in the plastic. Note: This brace is a direct food contact surface. The plastic lid, covering raw chicken in the Deli Walk in Cooler, is cracked on one edge and another corner is broke off.
- KDA_4_50111A - --There is rust build up on the shelf above the Meat Department cutting table and on the frame of the cutting table. There is 2 plastic roll bins, used to store single use items in the Deli, that have gray tape wrapped around the bottoms.
- KDA_4_50112 - --The cutting boards throughout the Deli Dept, have several scratches/gouges.
- KDA_4_50213A - --There was a tin can with grease build up , stored on the shelf below the Deli Prep Table. Note: An employee said this was used when they fry taco shells. Corrected on Site - Discarded
- KDA_4_60111A - --There were two luggers that were sticky to the touch, stored as clean below the steam table (in the Deli). Corrected on Site - Removed to be cleaned On the west storage shelf in the Deli, there was a plastic container storing salt that still had the manufacturer's bar code sticker on the side.
- KDA_4_60111C - --There is dust and grime build up in the vent hood above cooking equipment (Deli Dept). There is dust build up on the fan covers in the Produce Walk in Cooler. There is grime and food residue build up around the edges of the Produce wrapping machine. There is grime build up on the door seals to the Milk Walk in Cooler. The cabinet below the Deli steam table has grime and food particle build up.
- KDA_5_20311A - --There is no hand washing sink in the smoking shed (outside the facility on the west side). Note: The manager said that they do handle the meats during the smoking/cooking process and also apply sauce to them.
- KDA_6_20215A3 - --There is a 1/2" gap along the bottom of the double door (east side of storage area). There is gaps along both side of the load dock at the west overhead door (storage area).
- KDA_6_501111A - --There were 20 (fresh in appearance) rodent excreta pellets in the east storage closet in the Deli. Note: The facility has a professional company that treats for pests. Observed live traps throughout the facility.
- KDA_7_10211 - --There were 3 spray bottles with clear liquids stored on the shelf below the Produce Prep Table. One bottle had a manufacturers label (glass cleaner) and also had sanitizer hand written on the bottle. One bottle had no labeling and another was not legible. Note: An employee said he was unsure what they were. Corrected on Site - Discarded
