- KDA_3_30211A1b - --In the stainless steel RIC a tray of raw hamburgers was stored next to bags of RTE roast beef and ham. A pan of raw beef was stored on a pan of cooked RTE ham. A bowl of cooked potatoes was stored on a box raw shell eggs. In the make table raw containers of raw bacon, sausage and shell eggs were stored above RTE hash browns, milk and cheese. COS-items were rearranged.
- KDA_3_50118A1 - --In the SS RIC there was a bag of roast beef and a container of cheese sauce with an open date of 1/5. In the pizza make table there was a container ham that was dated as opened on 1/6. The items were held greater than days. COS-discarded.
- KDA_3_50118A2 - --In the SS RIC there was a bag of ham lacking a date mark. The PIC said it was opened on Sunday. In the make table there was an open jug of milk lacking a date mark. The PIC said it was opened yesterday morning (greater than 24 hours ago). In the pie cooler, cream pies lacked a preparation date. The PIC said they were cooked on Sunday. COS-items were dated.
- KDA_3_50118A3 - --In the pizza make table pizza sauce was dated 1/3-1/13. The cook said the items in the pizza make table are dated for 10 days. cOS- educated, items were re-dated.
- KDA_4_10111A - --The bucket used to transport ice to the soda machine is a non-food grade Wal-Mart bucket. In the dry storage room the bucket used to store powdered sugar is a non-food grade Wal-mart bucket. Inspector educated and advised to use food grade buckets.
- KDA_4_60111A - --The following items stored as clean had grease or food debris on them: measuring cups, a spatula, pie plates and bowls. Plastic food storage containers, stored as clean had stickers and sticker residue on them. COS-items were taken to warewashing.
- KDA_4_60211E4 - --There is a small amount of pink mold on the deflector in the ice machine. There is mold on the interior of the bucket that is used to transport ice to the soda machine.
- KDA_6_20214 - --The men's restroom door is not self closing.
- KDA_7_20111A - --At the wait station spray bottles of chemicals were stored next to and facing single use napkins, chocolate syrup and ketchup. In the retail area, a can of medicated first aid spray was stored on a wire rack above muffins and cup cakes. In the office area jugs of floor cleaner and bleach were stored above single use cups and lids. COS-items were rearranged.
