4-302.14 - --Located in the bar area, there are not any test strips for the quat sanitizer.
KDA_3_30212 - --Located in the kitchen there were 3 containers of food substance with no labels. PIC said one was flour, one was chili powder, one was garlic powder.
KDA_3_30412A - --Located in the rice container, there was a drinking cup with no handle being used as a scoop. COS. PIC placed cup in trash can.
KDA_3_30414B1 - --Located on the kitchen counter there was one wet wiping cloth laying on the counter and not in sanitize solution.
KDA_3_30512 - --Located in the mens toilet room, there were 10 rolls of single use paper towels being stored on a shelf. PIC said towels are used for hand drying. COS. Towels were moved.
KDA_3_50115A - --There were 5 pans of cooked ground beef sitting on the counter and no cooling device or temperature control was being used. Temperature of beef was 115F. PIC said it had been sitting there for about 1 hour. PIC moved beef to the RIC to finish cooling.
KDA_3_50115B - --There were 5 pans of cooked beef being cooled in the RIC with lids on the pans and not allowing it to adequately and rapidly cool down.
KDA_3_50116A1 - --Located in the steam table, the cooked rice was only 85.3F. Employee said it had been in there for 30 minutes. The pan was sitting inside another pan and not sitting close to the heat element. Employee said he does this because the rice dries out, however he said he always heats the food in the microwave to get it hot enough. COS. Educated staff. Bottom pan was removed. Rice was reheated to 165F
KDA_4_20211A2 - --There were two cracked lids being used to cover food containers. COS. Lids were placed in trash can.
KDA_4_60111A - --The table mounted can opener blade had dried food debris on the surface. Employee did not know when it was last washed. Item was placed at dish area to be washed.
KDA_4_60111C - --The floor fan in the dish area has a large accumulation of lent debris. The kitchen mixer has an accumulation of dried food debris. The vent hood filters have an accumulation of grease debris. The nozzle holder at the bar area has an accumulation of pink slimy mold.
KDA_4_60211E4 - --The ice machine guard plate has visible black mold on the edges.
KDA_5_20511B - --Located at the bar sink there was an old soap dispenser sitting in the sink and you have to move it to wash hands.
KDA_6_20211A - --There is one ceiling light in the kitchen dish area that is exposed and does not have a cover.
KDA_6_30111 - --Located at the hand sink at the bar, there was not any soap or other hand cleaning device. Employee said they had been using the toilet room sink to wash hands. COS. Soap was placed at sink.
KDA_6_50111 - --The ceiling lights in the kitchen area have cracks in them.
KDA_6_50112A - --The floor in the wait staff area has trash debris under the counter. The floor area under the soda syrup has a large accumulation of sticky residue and the room has a foul odor. The door to the office has an accumulation of black debris.
KDA_7_10211 - --There was one spray bottle of pink liquid with no label and one spray bottle of yellow liquid with no label. PIC said the pink was sanitizer and the yellow was disinfectant. COS. Bottles were labeled.
KDA_7_20111A - --Located in the bar area, there was one spray bottle of sanitizer sitting beside and touching one bottle of lime juice. COS. Chemical was moved.
KDA_7_20911 - --Located in the wait staff area, there was one bottle of hand lotion and one can of hair spray sitting on the shelf beside single use lids. COS. Items were moved.
