KDA_3_30211A1b - --Container of raw chicken in the MT stored above containers of cooked, RTE chicken. Cooked, RTE chicken was covered and no signs of contamination. PIC COS by moving the raw chicken.
KDA_3_30412B - --Bowl stored in the rice container for serving did not have a defined handle and was in contact with the rice. PIC COS by replacing it with a scoop.
KDA_3_30414B1 - --Damp wiping cloth stored on the front counter, not in a sanitizer solution.
KDA_3_50114D - --Raw Shell Eggs were stored on prep table, not under refrigeration. Eggs temped at 59.3. PIC said that they had been delivered this morning at about 11. PIC COS by moving the raw shell eggs to the WIC.
KDA_3_50115A - --Facility had several containers of cooked sweet and sour chicken sitting out cooling at room temperature. Educated PIC on the need to use proper cooling methods. PIC COS by moving chicken into WIC.
KDA_4_60111A - --A knife stacked and stored as clean on the clean side of the three compartment sink had visible food debris. PIC washed the knife without sanitizing to COS. Educated PIC on the need to wash, rinse, and sanitize and COS by having owner pull the knife to be washed, rinsed, and sanitized.
KDA_5_40211 - --Container of raw chicken was stored in the sink basin, below the flood rim level of the three compartment sink. PIC COS by moving the chicken above the flood rim level.
KDA_6_20214 - --Toilet room door is not self closing.
KDA_6_30114 - --Kitchen handwashing sink and toilet room handwashing sink did not have handwashing reminders. COS by providing stickers.
KDA_7_10211 - --PIC indicated that a container labeled as cooking wine was actually bleach. Container was not marked to indicate that it was not cooking wine. PIC COS by properly labeling the container as bleach.
KDA_Defacto - --Facility had sweet and sour chicken sitting out at room temperature to cool. Unable to determine time that items were at 135. Educated PIC and had items placed under proper cooling methods. --Facility uses a licensed pest control provider. Back door had visible gaps to the outside at the bottom corners but facility has an additional screen door on the inside that is tight fitting and in good condition. --Fryer: Egg Rolls 195. --MT: Shrimp 40

KDA_3_30412B - In-use utensil storage (Non-PHF/TCS-handles above food) During pauses in FOOD preparation or dispensing, FOOD preparation and dispensing UTENSILS shall be stored in FOOD that is not POTENTIALLY HAZARDOUS (TIME/TEMPERATURE CONTROL FOR SAFETY FOOD) with their handles above the top of the FOOD within containers or EQUIPMENT that can be closed, such as bins of sugar, flour, or cinnamon.
