- KDA_5_20515B - --Under the three compartment sink, the pipes are leaking. The faucet at the three compartment sink is leaking.
- KDA_6_50111 - --The floor through out the kitchen and dining room is missing the top coating, leaving the bare concrete. Person in charge said they have started scrapping what surface is on there to get ready to redo the flooring. The two kitchen tile that were missing is now replaced, with just two small holes in them.
- KDA_Defacto - --In the two door reach in, Aoli(egg and oil mixture) 34.6F, On the make table they have purchased a thermal unit with a lid that holds ice to hold their lettuce, cheese and pico sauce. The pico sauce was 35.4F.

- 3-304.15(A) - --The cook was preparing tacos, then used his gloves hands to pull up his jeans. The cook then grab to ready to eat taco shells with out washing his hands and changing gloves. COS- taco shells were discarded and gloves were changed and hands washed.
- 3-302.11(A)(2) - --In the reach in cooler, there was raw beef, raw pork on a pan with no separation. Sitting on top of the raw beef and pork was a container of raw chicken. Educated on separation of raw meats. COS- meats were separated.
- KDA_3_30212 - --On the shelf next to the hand washing sink, was a container with a white powder in it with put a label. Person in charge did not know what it was. COS-powder was discarded. On the same shelf, was another container of white powder with out a common name and a container with yellow liquid with out a common name. Person in charge said they were four and fryer oil. COS-containers were labeled with common name.
- KDA_3_50116A2 - --In the reach in cooler, made on site, pico sauce(not cooked)43.3F, person in charge said it was made at 9:30AM today. COS-placed in Dasani cooler. Tofu 43.3F. COS-Tofu was discarded, person in charge said it was to be discarded today. Date mark was OK. Quart of cream, 46.2F, 44.4F. The cream was sitting near the front where the doors were open and closed a lot today. COS-cream was discarded. White shredded cheese 44.4F, cheese was sitting near the front. COS-cheese was placed in Dasani cooler. In the make table, shredded lettuce 60.1F, pico sauce 48.3F. They had these items on ice, ice was only on the bottom. Educated on ice needing to be around the sides. Employee said lettuce had been out about half hour and pico about an hour. COS-Items were placed in a larger container and ice was added. Ambient temperature of reach in was 45.9F. The reach in was opened and closed a lot. The ambient at 3:15PM was 43F.
- KDA_4_20211A2 - --Stored on a shelf by the hand washing sink, was three rubber spatula's with pieces missing out of them. Stored above the three compartment sink, was one plastic lid that was melted. COS-these items were discarded.
- KDA_4_20311 - --They have two food thermometers, one was not at accurate temperature. I showed person in charge how to check and calibrate and make corrections. COS-calibrated and corrected.
- KDA_4_60111A - --On a shelf next to the hand washing sink was a pair of tongs stored as clean with dried on food debris. COS-tongs were placed at three compartment sink.
- KDA_4_60111C - --The handle on the reach in cooler had an accumulation of dried on food debris and grease. On the shelf by hand washing sink, the containers the serving utensils were stored in had food debris in them.
- KDA_4_90311B - --The bucket used to transport ice in, was stored on a hook with the ice contact surface face up. COS-ice was hung on hook inverted.
- KDA_5_20515B - --The handle on the Dasani reach in is missing. Person in charge said it broke today.
- KDA_5_50117 - --The unisex restroom does not have a covered trash can.
- KDA_6_50111 - --In the kitchen area, there are two ceiling tile that are missing. The floor through out the kitchen and dining room is missing the top coating, leaving the bare concrete exposed.
- KDA_7_20111B - --Above the ice machine were two spray cans of stainless steel cleaners, a spray bottle of all purpose cleaner. COS- items were moved.
- KDA_Defacto - --Educated on proper cooling methods. --Heated on the stove top, taco meat 176.6F. --In the reach in cooler, Queso cheese 41.4F, raw shell egg 41.6F, white shredded cheese 42.7F. Dasani reach in, cooked on site taco meat 41.5F. --In the rice warmer, rice 190.4F. In the steam table, shredded beef 147.8F. The cheese warmer, white cheese 140.2F.
