KDA_4_10111A - --Located in the RIC under the MT, there was a non food grade Sterilite container being used to store chicken. The chicken was in direct contact with the container. COS- moved chicken to another food grade container. KDA_4_60111A - --Located in the MT, there were containers of cut bell peppers and lettuce with stickers on the containers and lids. COS- moved containers to wash area, changed containers. KDA_4_60111C - --There is an accumulation of grease on the sides of the fryer. KDA_6_20214 - --Neither the men's nor the women's restrooms had self closing doors. KDA_6_20215A3 - --The back door in the downstairs basement has a one inch gap at the side where the door meets the door frame. KDA_6_501112 - --There were approximately fifty dead insects on the floor along the wall where the back door of the downstairs basement is. KDA_7_10211 - --Located on the floor under the three vat sink, there was an unlabeled spray bottle of purple liquid which the PIC identified as degreaser. COS- labeled.
