- KDA_3_30211A1a - --In reach in cooler, packages of raw beef were setting over fresh jalapenos. Corrected on site (COS) - meat moved to freezer.
- KDA_3_50115A - --A pan of cooked rice was setting directly on countertop, to cool per owner, for approximately 3 hours. Rice was 65F. COS - rice placed in smaller containers and put in reach in cooler.
- KDA_3_50118A2 - --In reach in cooler, an opened package of RTE sliced deli ham did not have consume by date. Owner said ham was purchased and sliced on 12/05/15. COS - ham dated.
- KDA_5_20515B - --In employee restroom, the hot water handle was broken, making the hot water difficult to turn on. NOTE: the employees are able to use the consumer restroom.
- KDA_7_20211A - --In storage room, a can of Hot Shot Flying Insect Spray, EPA 46515-48-8845, not approved for use in a food establishment. COS - owner placed can in personal vehicle to take home.
- KDA_Defacto - --Cold Holding: Reach in cooler - raw beef 39F. Refrigerator (front area) - 42.1F ambient. Make table - sliced ham 41F. Freezers ok. --Hot Holding: Steam container - refried beans 166F. --Wiping cloth sanitizer solution tested at 200ppm chlorine.
