5-202.14 - --The Watt's RPZ is leaking from the top and corroding. The person in charge called for it to be serviced on 9/8/15. The plumber came to inspect the device on 9/9/15 and ordered the replacement part the same day. There is a work order on file but the plumber is waiting on the part to be delivered. The establishment will contact KDA61 when the RPZ has been fixed and is working properly.

KDA_3_50116A2 - --In the make table 1 drawer (next to handsink), there was a plastic food grade container of chicken at 47.0F. It has been in the unit longer than 4 hours. COS-discarded. The ambient air temperature of the unit is reading 42.6F. In make table 2 there was a plastic food grade container of sliced tomatoes at 51.8F, a plastic food grade container of cut lettuce leaves at 47.8F and a plastic food grade container of diced tomatoes at 47.4F. The items have been in the unit longer than 4 hours. COS-discarded. In the make table 2 drawer there was a plastic food grade container of shredded cheese at 50.4F and blue cheese at 44.7F. They have been in the unit longer than 4 hours. COS-discarded. There was also a plastic food grade container of guacamole at 46.6F. It came from the WIC and then placed in the unit around 10:30am. COS-the PIC is going to use time for control until the unit can be serviced. The ambient air temperature of the unit was reading 48.0F.

KDA_4_204120 - --Make table 2 has water pooling at the bottom of the cooler and water is dripping on the floor.

KDA_4_60111A - --On the utensil rack there was four (1/2) plastic food grade containers with stickers/residue left on them and stored clean (holds bacteria). On the utensil rack above the food prep sink, there was one clear plastic food grade lid with dried food debris and stored clean (holds bacteria). COS-moved all items to the ware wash area.

KDA_5_20212A - --The men's restroom handsink was reading 73.3F and the women's handsink was reading 74.3F.
KDA_6_20215A3 - --The back exit door has a 1/2" gap along the bottom. This could allow the entrance for pests. The PIC has the door seal on order.
