3-501.17(A) - --In the WIC there was a ziplock bag of cooked ready to eat chicken breasts (made on site) with a make date of 10-22-15. It was pulled from the freezer and not date marked. COS-pulled from freezer on 1/27/16 and put date on the package. There were also two ziplock bags of cooked ready to eat pulled pork (mos) with a make date of 8-27-15 and 10-22-15. They were pulled from the freezer and not date marked. COS-pulled from the freezer on 1/27/16 and put dates on the packages.
KDA_3_10111 - --In the black ACE Refrigerator there was a plastic container of cooked corn without a known date. There was also a container of cooked green beans without a known date. COS-discarded both items.
KDA_3_20215 - --In the dry food storage on a shelf, there was one can of Campbell's Cream of Chicken with a deep dent on the bottom seam. COS-discarded.
KDA_3_30211A1b - --In the walk in cooler there were 2 metal sheet pans of raw ground beef being stored directly above an open box of ready to eat butter spread. There was also a ziplock bag of cooked ready to eat chicken breasts being stored directly next to a ziplock bag of raw ground beef. COS-rearranged items. (Note: no leakage detected).
KDA_3_30412E - --There were 3 plastic and 2 metal ice scoops being stored on top of the ice machine (not a sanitary surface). COS-placed on a clean metal sheet pan.
KDA_4_20211A2 - --In the dry food storage area, on a shelf there was a plastic food grade container of salt and the lid was cracked. COS-placed clear plastic wrap as a second barrier.
KDA_4_30212A - --There was no food thermometer available at the time of the inspection. COS-left a food thermometer.
KDA_4_50111A - --The ice machine located behind the bar is out of order, it is not producing ice (not in use).
KDA_4_60111A - --The large can opener that is connected to the counter had dried food debris on the blade and was not used today (can hold bacteria). COS-cleaned.
KDA_4_60111C - --There is dust debris on the fan blades inside of the alcohol walk in cooler.
KDA_4_60211E4 - --There is black mold behind the black plastic ice chute in the ice machine located behind the bar.
KDA_5_20515B - --The cold water handle at the 3 compartment sink located behind the bar leaks when water is supplied. In the men's restroom the right urinal is out of order due to it "salting up" (clogged). (Note: a plumber is coming on 1/28/16 to fix it).
KDA_7_10211 - --Below the handsink in the kitchen, there was a plastic bottle with green liquid and not labeled. It was Mean Green Degreaser. COS-labeled the bottle.
KDA_7_20111B - --On a shelf there were bottles of Floor & All Purpose Cleaner being stored directly above a roll of single use disposable paper towels. COS-rearranged items. (Note: no leakage detected).
