KDA_6_50112A - --There are 5 dead cockroaches in the wooden cabinet at the far end of the bar. COS by VD KDA_3_50118A1 - --The following RTE products were held over 7 days as shown: chopped cooked beef 11/3, sausage pizza topping 11/5, diced cooked chicken 11/5, Andouille sausage 11/5. COS by VD KDA_3_60311A - --They do not have any form of Consumer Advisory, although they do serve hamburgers and steaks "to-order". KDA_4_20211A1 - --The microwave oven used in the kitchen has the interior surface flaking and chipping off, exposing rusted metal underneath. The flaking, chipping paint finish could fall into food from the microwave ceiling. The interior cannot be properly sanitized in this condition. KDA_4_20211A2 - --Four utensils (2 spatulas and 2 ladles) being used for food preparation per Chef, have burnt, melted, pitted and split handles. COS by discarding KDA_4_60111C - --There is a build up of water from condensation in the bottom of the pizza make-table in the kitchen. KDA_6_20215A3 - --There is a light gap at the bottom of the rear exit door from the kitchen, which could allow entry of pests. KDA_6_501111A - --Three live cockroaches are in the facility - 2 in the kitchen/warewashing area above and below the 3-vat sink, and one in the back of a piece of equipment in the kitchen. There are dead cockroaches in the corners and behind equipment, in the dry storage room and the kitchen. Chef says that the owner is aware of the problem, and that he believes they are having the facility treated every-other-week.
