KDA_4_60111A - --Three cutting boards stored as clean had manufacturer's stickers on the boards (removed). Three plastic food containers holding sugar, powdered milk, and powdered sugar had stickers on the outside of the containers (employee started removing stickers for cleaning). One steel food pan stored as clean had masking tape on the outside of the pan (removed for cleaning). One sifter stored as clean had masking tape on the handle (had employee start removing for cleaning). Also, several single use plastic food container lids stored as clean and intended for reuse had stickers on them (removed).

KDA_5_20515B - --In the janitors closet next to the ice machine, the mop sink has a backflow prevention device built into the faucet (atmospheric vacuum breaker). This device has failed and leaks when the water is turned on. A hose is hooked to the faucet and it goes to a chemical dispensing unit, which has a built in backflow prevention device.
