- KDA_3_30512 - --There is a pair of boots sitting on a table top directly above a bag of onions sitting on the bottom shelf of this table top. COS - the boots were moved.
- KDA_3_50116A2 - --The cooler near the window is measuring 48.6 F ambient temperature after being left closed for 10+ minutes. The following items in the cooler are not within proper cold holding temperatures: White Egg Patty - 49.8 F Yellow Egg Patty - 61.0 F Milk - 48.8 F The manager said these items have all been in the cooler since last night. The temperature on this cooler was not taken this morning. She indicated that they've been having a few problems with the cooler that they're trying to get fixed. COS - the items were all discarded. Only items not requiring time/temperature control for safety are being stored in the cooler for the time being.
- KDA_4_30111 - --The cooler next to the drive thru window is measuring 48.6 F ambient temperature after being left closed for 10+ minutes. It was not open for extender periods of time prior to this temperature being taken.
- KDA_4_60111A - --On a table in the back prep area there is a pair of boots, a jacket, and a purse. When asked, an employee said the dishes (food service pans and lids) on this table were being stored as clean. The boots are touching the edge of a stack of pans and a stack of lids. COS - the pans and lids were moved to the 3 vat sink to be cleaned. The boots were moved and jacket.
- KDA_4_90311C - --In the back room there is a shelf next to the 3 vat sink that has single-use plastic containers on the top shelf. These containers are outside of their original protective packaging and are neither covered nor inverted.
- KDA_5_20212A - --The hand sink in the front (near the make table and window) is measuring 75.2 F after running for 5+ minutes. Later on in the inspection, the sink reached 108.7 F. It then cooled back down below proper hand washing temperatures again. The manager has called a plumber to come look at this issue. All other sinks, including the hand sink around the corner by the ware washing sink, are working appropriately.
- KDA_5_40211 - --There were bags of pickles, jalapenos, banana peppers, and black olives sitting on the ledge of the 3 vat sink where dirty dishes are placed. They are within the flood rim of the sinks which are directly plumbed to the sewer line. The far right vat of the sink (the sanitizing vat) is indirectly plumbed to the sewer line, but currently is full of sanitizer with dishes drying on the ledge of the sink. Recommended to manager that they merely put these items in clean containers in the future that will stand above the edge/flood rim of the sink if they need to set them out temporarily. COS - the items were dumped into plastic food containers and stored in coolers.
- KDA_6_20213B2 - --There is an insect control device mounted on the wall directly above clean dishes that are drying on the ledge of the 3 vat sink.
- KDA_6_30112 - --The hand sink in the ware washing area does not have a supply of paper towels available. Corrected on Site (COS) - the manager is supplying with paper towels.
- KDA_7_20811B - --There is a first aid kit on a shelf in the prep area that has containers of tomatoes sitting directly beneath it/directly beneath the first aid kit. The kit is also sitting directly on top of a container with a food saw/meat cutter inside. COS - the first aid kit was moved.
- KDA_Defacto - --Quaternary Ammonia test strips were provided. --Steam Table: Meatballs - 154.8 F --The sanitizing vat of the 3 vat sink has Quaternary Ammonia sanitizer in it measuring 100ppm. The label on the bottle calls for 150-400ppm. This sanitizer was not in use when the concentration was taken. Notified staff member that the sanitizer needs to be remade. --Walk-in-Cooler: Meat - 41.6 F, Sandwich Make Table: Steak - 42.3 F, Cooler storing spare items for the make table: Ham - 39.9 F, Windowed Cooler in dining area: Milk - 43.3 F
