KDA_3_10111 - --There was an opened bag of hashbrowns in the walk in freezer with ice debris along the top and inside the bag. COS- PIC voluntarily discarded.
KDA_4_50111A - --One of the fan guards/fans in the walk in freezer is broken and there is excessive build up of ice on the tubing and racks.
KDA_4_50111B - --The handle on the reach in hot hold unit (top) is broken.
KDA_4_60111A - --There were approximately 3 serving ladles hanging on the clean utensil rack with old food debris along the food contact surfaces. COS- PIC moved all three ladles to the 3 vat for additional cleaning.
KDA_4_60111C - --There is excessive ice build up on the tubing and racks in the walk in freezer do to a broken fan/fan guard.
KDA_4_60211E4 - --There was old syrup residue around the soda dispensing nozzles and on the push handles.
KDA_6_20215A3 - --There is a quarter inch gap in the lower left hand corner of the back exit door large enough to allow the entry of pests.
KDA_6_50112A - --There is standing water around the hot water heater by the back door.
KDA_7_20111B - --There was a box of hand sanitizer on the rack in the back room stored directly above boxes of RTE syrups for the soda unit. COS- PIC moved the chemical to the other rack.

No activity. --Wiping cloth QT measured at 200ppm on line and at front counter.
