KDA_4_20111 - --The white reach in freezer (to the right of the Hotpoint RIF) door handle is broken. KDA_4_20211A2 - --In the clean dish strainer there was plastic tongs with melted edges (not easy to clean). On a shelf to the left of the Traulsen RIC there was one plastic container that had cracked edges. On a rack near the decorating station there was one green plastic tub with a broken edge and had silver duct tape around the container. COS-discarded. KDA_4_60111A - --The large Hobart mixer had dried food debris on the metal surface where the whisk is inserted and was stored clean. COS-an employee cleaned it. KDA_4_90311A - --There was one box of foam cups being stored directly on the floor in the basement. KDA_6_20215A3 - --There is a 1/4" gap along the left and right side of the back exit door. This could allow the entrance for pests. KDA_6_50111 - --There are broken floor tiles in the kitchen which is exposing the wood floor underneath (not easy to clean). The handsink in the men's restroom is cracked and broken on the right side. KDA_6_501111A - --There are 10 live flies around the 3 vat sink in the back room and 5 live flies near the decorating station. (Note: have a contract pest control provider). KDA_6_50112A - --There is trash on the floor in the basement storage area. KDA_7_20111A - --On a shelf there was a working spray bottle of Grease Lightening Cleaner & Degreaser being stored directly next to a container of Garlic Powder. COS-removed chemical. KDA_7_20111B - --There were 6 jugs of bleach and 2 working spray bottles of Grease Lightening Cleaner & Degreaser being stored directly on top of the Hotpoint reach in freezer (food in unit).
