- 5-202.14 - --At the bakery mop sink a wye splitter was attached downstream of the AVB. One hose was attached to a chemical dispenser with a built in back flow prevention device on it. The other hose was below the flood rim of the sink. COS-the hose was cut to above the flood rim.
- KDA_3_30211A1b - --On a retail shelf packages of raw Butterball turkey bacon were stored in contact with packages of Oscar Mayer fully cooked turkey bacon. COS-a divider was placed to separate.
- KDA_4_60111A - --In the deli area there were pans stored as clean with food debris on them. In the bakery there was a spatula with a sticker on it. COS-items were moved to be washed.
- KDA_5_20212A - --One deli hand sink hot water measured 82F. The other deli hand sink measured 92F. The meat department hand sink measured 88F. COS-The PIC turned off the cold water and the sinks then measured above 100F. A plumber has been called to look into the situation.
- KDA_5_20515B - --At the mop sink by the dairy department there was a wye splitter attached to the faucet downstream of the AVB. The hose was above the flood rim.
- KDA_7_20111B - --At the bakery 3 vat sink, the bulk bag of sanitizer was stored directly over clean dishes. COS-chemical was moved.
