5-202.14 - --On the dishwasher pre wash sink, the atmospheric vacuum breaker serving the water line to the garbage disposal unit (located under the sink), has failed and leaks when the water is turned on. KDA_3_50113A - --Raw hamburger was being thawed at room temperature. KDA_4_20211A2 - --On a rack, one plastic scoop was cracked, this prevents proper cleaning and pieces of plastic can end up in foods (discarded). KDA_4_60111A - --Five single use plastic food container lids, stored as clean and intended for re-use, had remains of stickers and adhesive on the tops of the lids (discarded). KDA_4_70211 - --An employee cleaned the interior of a large stainless steel kettle by hand. It was cleaned with soapy water, then wiped down with fresh water and a cloth. There was no sanitizing step afterwards (reviewed proper procedure. KDA_6_20215A3 - --At the bottom corner of the east exterior kitchen door, is a gap of about 1/2 inch, large enough for pest entry. KDA_6_501111A - --Fresh rodent droppings (numbers approximate): in dry storage four on the floor and 30 on top of a bag of brownie mix. As per the person in charge, they have a hired pest control service that sets out bait stations. Pest control method observed was one rodent bait station in the dry storage. KDA_6_50112A - --Old rodent droppings on the floor behind the kitchen water heater tank (fresh droppings in dry storage were being cleaned up just prior to and during the start of the inspection).
