- KDA_5_10311B - --No hot water at the bar handsink, the mop sink, and the bar 3-vat sink (the restrooms have hot water at the handsinks). A pilot light on a water heater tank was found to be unlit, an employee tried to light it but it would not work. The hot side water at the bar handsink was 55 degrees, the mop sink 57 degrees, and the bar 3-vat 56 degrees.

- KDA_6_20114A - --Carpet is the floor material in two restrooms by the showers and handsinks.
- KDA_6_20215A3 - --The south exterior door has a gap at the bottom of about 1/2 inch, large enough to allow pest entry.
- KDA_6_30111 - --No soap at the bar handsink (soap placed at sink).
- KDA_6_501112 - --A dead roach is in the bottom of the stainless steel reach in cooler. Dead insects are throughout the facility including on window sills. Some of the dead insects are roaches.
- KDA_6_501115A - --At the start of the inspection, a live domestic cat was in the bar dining area on a seat (removed cat from building).
- KDA_6_50112A - --Debris is on the floor throughout the facility, primarily by the walls. Old rodent droppings are in a cabinet under the handsinks in the men's restroom and in the small north entryway room just of off the old unused kitchen. Dried feces is built up on a toilet stool in the men's restroom in the bowl and back section of the toilet seat.
- KDA_7_20111A - --On a shelf at the bar, bleach water, glass cleaner, and air freshener stored next to coffee, a spoon, and Styrofoam cups. The bleach water container was touching the Styrofoam cups (moved).
- KDA_7_20212A2 - --Unapproved D-Con rodent bait set out in the old unused kitchen, EPA number 3282-65 (discarded).
- KDA_7_20612 - --The unapproved D-Con rodent bait was set out in the open in the original cardboard box (discarded).
