There was also another plastic food grade container of BBQ pork dated marked 11/29-12/6. Both items were date marked past 7 days. COS-date marked the items with the correct dates.
- KDA_4_20211A2 - --In the Frigidaire RIF there were 2 plastic food grade containers (Roundabouts & Chicken Fried Chicken) with broken edges (physical hazard). COS-placed both food items in new containers.
- KDA_4_60111A - --On the utensil rack there was one small round plastic rubbermaid container with a sticker left on the outside and stored clean. There were also 3 medium sized rectangle plastic rubbermaid containers with stickers on the outside and stored clean. COS-moved the containers to the ware wash area.

- KDA_3_10111 - --In True RIC 2, there was a glass measuring cup with mashed potatoes without a date. The cook was not sure when it was made. COS-discarded.
- KDA_3_50116A2 - --In the make table there were plastic food grade containers of cheddar cheese at 56.0F, american cheese at 50.7F, pepperjack cheese at 49.2F and swiss cheese at 43.7F. These items have been in the unit longer than 4 hours and were above the fill line. The ambient air temperature of the unit was reading 28.2F. COS-discarded the cheese. In the Dr. Pepper RIC, there was a gallon of milk at 44.1F, 1/2 a gallon of milk at 45.4F and another 1/2 gallon of milk at 44.3F. There was a packet of butter (keep refrigerated) at 49.3F. There was a packet of whipped spread (keep refrigerated) at 48.2F. There was an open bag of romaine salad at 49.1F and a large plastic food grade container of cut salad at 49.7F. All of these items have been in the unit longer than 4 hours. The ambient air temperature of the unit was reading 44.6F. COS-discarded all potentially hazardous food from the unit.
- KDA_3_50118A1 - --In the walk in cooler there was individual bags of diced ham date marked 11/16-11/22 (held past 7 days). COS-discarded.
- KDA_3_50118A2 - --In the Dr. Pepper RIC there was an open bag of romaine salad and a large plastic food grade container of cut salad without dates. The person in charge said they were opened on 11/21/15. COS-discarded due to the salads being held above 41F for more than 4 hours.
- KDA_4_20211A2 - --In the Frigidaire RIF there were 2 plastic food grade containers (Roundabouts & Chicken Fried Chicken) with broken edges (physical hazard). On the utensil rack there were 3 large plastic rubbermaid lids with broken edges. COS-discarded.
- KDA_4_30111 - --The Dr. Pepper RIC ambient air temperature is reading 44.6F. (Note: the person in charge called for the unit to be serviced this afternoon).
- KDA_4_60111A - --There was one metal ladle hanging above the stove with dried food debris on the backside of the handle and stored clean. COS-moved to the ware wash area. On the utensil rack there was one 3.5 gallon plastic food grade container with stickers left on the outside and stored clean. COS-moved to the ware wash area. On a shelf in the storage area there were 2 plastic rubbermaid containers with clear tape and labels left on them, stacked and stored clean. COS-removed the tape and labels and moved to the ware wash area.
- KDA_5_20515B - --The hot water handle leaks when water is supplied at the 3 compartment sink located at the bar.
- KDA_6_50111 - --There are broken floor tiles throughout the kitchen and in the dry food storage area.
