  • KDA_6_501111A - --Repeat violation - fresh rodent droppings (numbers approximate): 12 in a cabinet under the 7-Up reach in cooler, six in a cabinet under the counter handsink, 14 on the floor in dry storage behind a box of single use Styrofoam food containers, seven on the floor in the southwest corner of the warewashing room, and one on the bottom shelf of the black plastic shelving unit in the warewashing room next to a box of soda syrup. As per the person in charge, they have a hired pest control service that sets out rodent bait. Pest control methods observed were rodent bait stations, sticky traps, mechanical traps, and the Contrac rodent bait set out in bags.
  • KDA_7_10211 - --Repeat violation - in the warewashing room, one unlabeled spray bottle containing a blue liquid (windex as per the person in charge)(labeled).
  • KDA_7_20612 - --New violation - two bags of Contrac rodent bait (EPA number 12455-856)(approved for use in a food establishment)were set out in the open. One bag was in a cabinet under the counter handsink, and the other bag was in a cabinet under the 7-Up reach in cooler (the person in charge removed the two bags). Advise talking to the person who set these out and see if there any other bags. If there are, remove or put in an approved covered tamper resistant container.

  • KDA_4_20216 - --Knives are stored in a wooden block in the food prep area, these are prohibited.
  • KDA_4_90311C - --Single use pizza boxes are stored on the floor in the food prep area.
  • KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14.
  • KDA_6_20215A3 - --The east exterior door has a gap at the bottom of about 3/4 inch. large enough for pest entry.
  • KDA_6_501111A - --Fresh rodent droppings (all numbers approximate): one in a cabinet at the cashier's stand under the lottery machine and in dry storage, two on a box lid and 30 on the floor under the electrical boxes (that are on the north wall). As per the person in charge, they have a hired pest control service that sprays and sets rodent bait stations outside. Pest control methods observed were one sticky trap, and rodent bait stations inside and outside of the building.
  • KDA_6_50112A - --Not cleaning up rodent droppings. Also, old rodent droppings are in the walk in cooler floor next to the walls. In the ware-washing room, old rodent droppings are on the floor near the west wall and soda syrup is on the floor under the syrup storage rack.
