  • R/- the interior of the fryer cabinets, and floor under the fryers was soiled
  • - hood unit above the char broiler was soiled. Reportedly this area was scheduled to be serviced in the near future.
  • - ceiling above the coffee machine was soiled/stained
  • - floor surface and floor/wall juncture under the counter is soiled
  • - floor drain under the pop syrup box rack was soiled
  • - continue to repair the ceiling surface above the pop syrup rack which is in disrepair
  • R/- lacking or soiled caulk between the floor tiles throughout, especially in the fryer area
  • - floor surface under the drive-thru pop dispensing machine was soiled
  • Clean or repair these areas.
  • 6.2 Provide 10 foot-candles of lighting in the freezer unit currently 2-4 foot candles.
  • Note: Operation added a Silver King freezer unit model number SKMWF34 with ETL listing, and a manual chopper and dicing machines. Contact this office at 330-723-9523, when equipment is added to the operation. Business card was provided today.

As discussed during the last standard inspection with PIC. Provide this office with and updated list of menu items that this operation use a frame of time instead of temperature for holding the foods. It appears this office has approved breakfast menu items but is lacking in lunch and dinner menu items.
Discussed cooling and holding guidelines for the iced coffee.
  • *4.5/4.6 Observed the sliding trays for the Duke hot holding unit were stained or soiled, water dispensing nozzles on the self service machines contained mineral deposits, Adkins thermometer probe and unit were soiled, one container on the storage rack was soiled and the lemonade nozzles was soiled. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch. Temperature measuring devices shall be washed and sanitized prior to use and storage.

  • R/- door seal on the one door freezer unit was soiled and damaged
  • R/- the handles, interior, and floor under the cabinets on the fryers was soiled
  • R/- caulk at the wall hand sink juncture was lacking
  • - hood unit including the screened areas, and the top vents on the fryers contain grease accumulations
  • R/- ceiling vent by the storage area is soiled.
  • - leaky Icee Machine
  • - floor surface under the self service beverage counter is soiled
  • R/- damaged ice machine buttons on the self service machines
  • - lacking or soiled caulk between the floor tiles throughout

TIME AS A PUBLIC HEALTH CONTROL: Onion rings were holding at 84°F at room temperature, also chicken fries were holding at 71°F at room temperature. Additionally, an extra pan of American cheese contained an air temperature of
49°F and was not marked with a time. All food items are using a frame of time instead of temperature. Submit to this office the procedures for handling these food items for approval.
  • R/*4.5/4.6 Observed the sliding trays for the Duke hot holding unit were stained or soiled, equipment on the storage rack such as but not limited to were soiled: sliding trays from the Duke hot holding unit were soiled. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch. Continue to removed grease accumulations from these areas.
  • R/6.4 Repair the damaged caulk in the mop sink area, to provide a cleanable surface.
  • R/4.5/6.4 The following areas were soiled:
  • - handles on the cabinets on the fryers
  • - ceiling vents as needed. Reportedly new vent is on order.
  • - top shelf of the wired rack contains food accumulations
  • Clean these areas.
  • R/4.4 The following areas were damaged:
  • - damaged containers throughout
  • - both ice buttons on the beverage dispensing machines, reportedly on order.
  • Repair these areas to provide a cleanable surface. Continue to replace containers.

  • *4.5/4.6 Observed the sliding trays and the grates for the Duke hot holding unit were stained or soiled, equipment on the storage rack such as but not limited to were soiled: crock, sliding trays from the Duke hot holding unit, and a container were soiled. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • 6.4 Repair the damaged caulk in the mop sink area, to provide a cleanable surface.
  • 4.5/6.4 The following areas were soiled:
  • - door seal on the one door freezer unit
  • - the handles, exterior, and floor under the cabinets on the fryers,
  • - ceiling tiles and vents as needed including in the restrooms
  • - caulk at the wall hand sink juncture
  • - hood units and devices under the hood contain grease accumulations
  • - exterior of monitors and supports area soiled
  • - all Duke hot holding cabinets are soiled and/or contains food debris
  • - caulk and wall surface above the three vat sin
  • - top shelf of the wired rack contains food accumulations
  • - floor surfaces under the ice machine and beverage station in the drive thru area
  • - floor/wall junctures throughout the operation as needed
  • Clean these areas.
  • 4.8 Equipment was stacked wet on the equipment rack. Air dry equipment prior to storage.
  • 5.4 Clean the excessive garbage in the dumpster area, and keep the dumpster lids closed to prevent pest entry.
  • 4.4 The following areas were damaged:
  • - top of the Cres-core unit contains a black surface that is uncleanable
  • - damaged containers throughout
  • - the interior grey edge on the freezer unit
  • - both ice buttons on the beverage dispensing machines
  • Repair these areas to provide a cleanable surface.
  • 6.2 Observed 5 foot candles on lighting in the walk-in freezer. Provide 10 foot-candles of lighting throughout.
  • 5.1 Metal ring surrounding the three vat faucet is damaged. Plumbing shall be maintained in a state of good repair
11/30/2010Standard2These foods were proper marked for time in lieu temperature today.
TEMPERATURE MEASURING DEVICE: Device was non-functioning, due to a dead battery. The battery on the device was changed today. Temperature measuring devices shall be accurate within +/- 2°F to monitor temperatures of potentially hazardous foods.
HOLDING: Observed the whipped butter were holding at room temperature (73°F). Potentially hazardous foods shall be maintained at 41°F and below to limit the growth organisms. Discussed with PIC to relocate these items in the refrigerator.
