- *4.5/4.6 Observed ice machine shield and pop nozzles were soiled and contained a mold-like residue. Food contact surfaces shall be washed, rinsed, and sanitized and clean to the sight and touch.
- R/6.4 Clean the floor more thoroughly throughout which appears to be stained or soiled.
- 6.4/4.5 The following areas were soiled:
- - Walls in the walk-in cooler appear to contain a mold-like growth
- - Interior of the True reach-in cooler
- - Door seals and surrounding areas on the door frame on the True freezer unit contained a mold like growth or ice accumulation.
- - Floor surface surrounding the floor drains under the preparation and three-vat sink
- - Top of the grease trap contained pooling water
- - Walls in the storage are contained food splash
- - Floor under the pop syrup rack - Cabinet under the pop dispensing machine
- - Interior of the milk cooler. Clean these areas.
- 6.4 Repair the damaged threshold at the side delivery.
- Note: Currently the operation removed pizza. In the next several weeks the menu will be evaluated, submit any changes to this office. Business card was provided.

- 6.4 Clean the floor more thoroughly in the following areas:
- -R/debris under the 3-compartment sink
- -debris and dirt under the freezers
- -debris and dirt under the pizza prep table
- Clean these areas.
- R/3.5 Observed a price tag covering the expiration date on Chef Boyardee Mac N Cheese and Ravioli. Do not conceal manufacturers dating information.
- 3.5 Provide labels and ingredients for all of the foods in the self-serve pastry case.
- NOTE: Due to heavy rain yesterday, the lights are out in the back room (dry storage, freezers/cooler, and dish washing).
- Manager stated that repairs are scheduled for later today. The freezers and reach-in cooler are functioning properly. No food prep is being done today.

- C/R/*7.1 Observed starter fluid stored beside Dart beverage lids and above Bunn coffee makers in the back room. Store toxic chemicals separate from and below food and equipment in order to prevent possible contamination.
- C/*7.0 Observed the following chemicals lacking a label:
- -windshield wiper fluid containers under the hand sink
- -blue liquid in a pump dispenser on the chemical storage rack
- Label all toxic chemicals in order to prevent confusion or misuse.
- *3.4 Observed breakfast pizza hot holding improperly at 130°F. Maintain potentially hazardous foods (PHFs) hot holding at 135°F or above in order to limit bacterial growth. PIC turned up the temperature of the hot holding unit. Reportedly, the pizza was cooked up to 150°F in the pizza oven.
- *7.2 Observed the following chemicals stored improperly on the retail shelves:
- -Tide laundry detergent above gallons of water
- -starter fluid above paper towels, salsa, bean dip, and Reynolds Wrap foil.
- Store toxic chemicals separate from and below food and equipment in order to prevent possible contamination.
- 4.4 Repair the torn door seal on the walk-in cooler door that exits into the restroom hallway.
- R/4.8 Observed 2 boxes of Dart cups stored on the floor in the back room. Store equipment 6 inches off of the floor in order to prevent possible contamination.

- NOTE: Observed Corn Nuts-Original on the retail shelf beyond the expiration date of March 2011. Check and rotate stock regularly.
