  • 4.5 The following equipment needs cleaned:
  • -grease and congealed oil in the unused table top fryer
  • -dusty large beer shelving in the beer walk-in cooler
  • Equipment shall be free of dust, dirt, and debris and be clean to the sight and touch.
  • 6.4 Observed water damaged ceiling tile in the retail area. Replace these and repair any roof leaks.
  • NOTE: Observed the following foods for sale beyond the manufacturers date: Ragu (various) July-Aug 2011; Heinz Gravy Dec

  • R/*3.4 No date mark observed for the opened deli meats in the deli cooler. Ready-to-eat, potentially hazardous foods (PHFs) shall be dated the day of opening or preparation and used or discarded within 7 days. Recommend discarding any contaminated food.
  • *4.5/4.6 Observed dried food debris around the slicer blade. Food contact surfaces shall be washed, rinsed, and sanitized and clean to the sight and touch.
  • *3.0 Observed home canned pickles for sale in the operation. Foods shall be from an approved source. Provide documentation that the pickles are from a Dept of Agriculture approved operation (canning license) prior to offering for sale.
  • 4.5 The following equipment needs cleaned
  • -R/store fryer clean and free of grease build-up
  • -dusty beer walk-in shelving (6 pk Rolling Rock thru Bud Light)
  • Equipment shall be free of dust and debris and be clean to the sight and touch.
  • C/R/*7.2 Observed the following chemicals stored improperly:
  • -windshield washer fluid stored above styrofoam trays and napkins on the retail shelf
  • -lighter fluid stored above Schweppe's beverages on the retail shelf
  • Store toxic chemicals separate from and below food and equipment in order to prevent possible contamination.
  • R/4.1 Observed several house hold pieces of equipment throughout the operation. When this equipment fails to meet the requirements of the Ohio Food Safety Code it must be replaced with NSF, commercial equipment.
  • 4.4 The following equipment was damaged:
  • -R/peeling cover on the ice machine
  • -peeling material in the back of the open Smirnoff Ice case
  • Repair these in order to provide a more cleanable surface and keep equipment in good repair.
  • R/6.4 Observed some water damaged ceiling tile. Repair this.
  • R/3.5 Observed several jars/cans with the expiration date covered by price tags. Do not conceal manufacturers dating information.
  • NOTE: Observed the following foods for sale beyond the manufacturers expiration date:

  • -One accurate long stem thermometer available for use.
  • -Observed proper cold holding temperatures at 41°F or below.
  • -Observed proper raw egg and raw bacon storage, separate from and below ready-to-eat foods in the retail cooler.

  • -raw bacon above Arizona beverages in the open dairy case
  • -raw shell eggs stored on top of the cheese slices in the open dairy case
  • Store raw animal foods separate from and below ready-to-eat foods in order to prevent possible cross-contamination.
  • *4.5/4.6 The following food contact surfaces were soiled:
  • -R/grease build-up on the fryer basket
  • -syrup build-up on the Siberian Chill nozzles
  • Food contact surfaces shall be washed, rinsed, and sanitized and clean to the sight and touch.
  • R/*7.1/7.2 Observed the following chemicals stored improperly:
  • -windshield washer fluid stored above Schweppe's and Canada Dry on the retail shelves
  • -C/spray chemical stored on Kodak shelf above beverages
  • Store toxic chemicals separate from and below food and equipment in order to prevent possible contamination.
  • *3.4 Unable to locate the date marking book for the ready-to-eat(RTE), potentially hazardous foods(PHFs) in the deli cooler. RTE,
  • PHFs shall be dated the day of opening or preparation and used or discarded within 7 days.
  • R/4.1 Observed several house hold pieces of equipment throughout the operation. When this equipment fails to meet the requirements of the Ohio Food Safety Code it must be replaced with NSF, commercial equipment.
  • R/6.2 Replace the burned out lights in the walk-in beer cooler to provide 10 foot candles of light and in the Hard Cider case.
  • 6.2 Replace the missing light in the Beck's cooler.
  • 4.4 The following equipment was damaged:
  • -R/peeling cover on the ice machine
  • -peeling material in the back of the open wine case
  • Repair these in order to provide a more cleanable surface and keep equipment in good repair.
  • 6.4 The following areas need cleaned:
  • -floor below equipment shelves in the dish washing area
  • -floor below shelving in the walk-in beverage cooler
  • Physical facilities shall be maintained in good repair.
  • 3.2 Store the boxes of Snyder potato chips 6 inches off of the floor in order to prevent possible contamination.
  • R/6.4 Observed missing and water damaged ceiling tile throughout. Repair this.
  • R/3.5 Observed several jars/cans with the expiration date covered by price tags. Do not conceal manufacturers dating information.
  • 6.4 Observed employee food items in the open dairy case. Store employee personal items in a separate designated area.
  • 6.2 Display an Employee Must Wash Hands sign in the customer rest room in order to encourage proper hand washing.

  • (PHFs) at 41°F or below in order to limit bacterial growth. Maintain coolers holding PHFs at 41°F or below. Maintain PHFs on the bottom shelf, which has an ambient air temperature of 39°F until repairs/adjustments are made to the top shelf area.
  • C/*3.2 Observed cartons of raw shell eggs stored beside and above ready-to-eat foods in the deli walk-in cooler. Store raw animal foods separate from and below ready-to-eat foods in order to prevent possible cross-contamination.
  • *4.5/4.6 The following food contact surfaces were soiled:
  • -grease build-up on table top fryer basket
  • -residue on the ice machine shield
  • Food contact surfaces shall be washed, rinsed, and sanitized and clean to the sight and touch.

  • 3.5 Observed Ragu spaghetti sauce and salad olives with expiration dates concealed by price tags. Manufacturers dates shall not be concealed.
  • 6.4 Observed water damaged ceiling tile in the retail area. Repair and leaks and replace the damaged ceiling tile.
  • 4.8 Observed exposed coffee stirs at the self-service coffee counter. Provide wrapped stirs or a dispenser to prevent possible contamination.

  • -newly added hot plates

  • Equipment shall be commercial-grade (NSF or equivalent).

  • R/*3.4/4.4 Observed deli meats on the top shelf of the deli cooler cold holding improperly at 43°F. The ambient air temperature of the deli cooler top shelf area was 42.6°F. Maintain potentially hazardous foods (PHFs) at 41°F or below in order to limit bacterial growth. Maintain coolers holding PHFs at 41°F or below. Relocate PHFs to the bottom shelf, which has an ambient air temperature of 39°F until repairs/adjustments are made to the top shelf area.
  • R/*3.2 Observed a carton of raw shell eggs stored beside and above ready-to-eat foods in the deli walk-in cooler. Store raw animal foods separate from and below ready-to-eat foods in order to prevent possible cross-contamination.
  • R/*3.4 Observed the following foods lacking a date mark or held beyond 7 days:
  • -ham, salami, pepperoni, and bologna lacking a date mark
  • -roast beef discard date 9/6
  • -turkey discard date 9/8
  • Ready-to-eat, PHFs shall be dated the day of opening or preparation and used or discarded within 7 days. A date log was available, however, the items listed above were missing or shown to be beyond the discard date.
  • *4.5/4.6 The following food contact surfaces were soiled:
  • -grease build-up on table top fryer basket
  • -R/food debris build-up on the deli meat slicer blade
  • -residue on the ice machine shield
  • Food contact surfaces shall be washed, rinsed, and sanitized and clean to the sight and touch.
  • *3.2 Observed a can of Liberty pineapple slices with a dented seam on the retail shelf. Do not use dented canned goods in order to prevent possible contamination. Store damaged canned goods in a separate designated area for return/credit.
  • *7.2 Observed Lysol and other cleaners stored above Styrofoam compartment trays on the retail shelf. Store toxic chemicals separate from and below food and equipment in order to prevent possible contamination.
  • R/4.4 Repair the peeling ice machine exterior in order to provide a more cleanable surface.
  • R/6.2 Observed 1.3-14.3 foot-candles of light in the beer walk-in cooler. (14.3 observed in front of the door and less than 10 observed throughout) Provide 10 foot-candles of light throughout the cooler.
  • R/4.5 Clean the larger beer shelving in the walk-in cooler in order to remove dust build-up.
  • R/3.5 Observed Ragu spaghetti sauce and salad olives with expiration dates concealed by price tags. Manufacturers dates shall not be concealed.

  • NOTE: Observed the following food for sale beyond the expiration date:

  • NOTE: Observed personal milk and half and half in the open retail case. Store personal items in a separate designated area in order to avoid confusion.
  • NOTE: Observed milk dated 9/6/10 in a crate at room temperature next to the dairy case. Be sure to discard product.
  • NOTE: Minor changes were made to the operation. Upright freezers were removed and equipment was added. New equipment was added, including but not limited to:
  • -coffee dispensers
