Maintain coolers with potentially hazardous foods (PHFs) at 41°F or below in order to limit bacterial growth. PIC adjusted the temperature and the unit reached 41°F. The PHFs were still cold holding properly below 41°F.
  • C/4.8 Observed a squeeze bottle of salt lacking a label. Label all unidentifiable substances in order to prevent confusion or misuse.
  • 4.4 Observed a small tear in the Victory cooler's door seal. Repair this.
  • 6.4 Observed employee aprons, coat, and bag stored on the dry goods shelf. Store employee personal items in a separate designated area in order to avoid possible contamination.

  • *4.5/4.6 The ice machine shield is soiled. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • 4.5 Clean the following areas:
  • - the pooling water in the milk cooler
  • - soiled floor drain under the preparation sink
  • - interior of the steam table wells which contain calcification
  • - dusty hood unit above the dish machine
  • Correct these areas.
  • 6.4 Repair the damaged wall surface in the dish area, to provide a cleanable surface.
  • 4.8 Store the case of gloves, six inches off the floor to facilitate cleaning and to prevent contamination.
  • 4.1 Provide a thermometer in the memory care refrigeration unit to assure 41°F and below is maintained.
  • Note: The Memory care unit at this facility remodeled the kitchen area. The remodeled area includes a household Maytag dish machine with NSF household certification. Equipment shall be washed, rinsed, and sanitized, in the commercial dish machine in the kitchen area, or provide a commercial dish machine in this area. Reportedly dishes are returned to the kitchen for cleaning and sanitizing. Non-commercial refrigeration unit in the Memory care unit was checked and holding at 37°F. This refrigeration unit is used for snacks for the residents and an occasional meal for resident who missed a meal.

DATE MARKING: Observed cream cheese was dated 10/5/11. Ready-to-eat foods not used in 24 hours shall be dated the day of preparation and shall be used or discarded in seven calendar days. Educated PIC on this requirement.
Discussed cottage cheese was opened after the recommend sell-by/use-by date on the product.
TEMPERATURE MEASURING DEVICE: Observed 2 of the 3 temperature measuring devices were inaccurate.
Temperature measuring device shall be accurate within +/-2°F to monitor temperatures of potentially hazardous foods.
This inspector calibrated these devices today.
  • 6.4 Repair the damaged wall surfaces throughout, to provide a cleanable surface.
  • 4.8 Store the oven rack six inches off the floor to facilitate cleaning and to prevent contamination.
  • 4.5 Clean the ice accumulation in the Douwe Egberts coffee machine.
  • 6.4 Repair/clean the soiled caulk at the wall/scrap sink juncture, to provide a cleanable surface.
  • 4.4 Repair the damaged door seal on the Victory upright unit, to facilitate with temperature maintenance.
  • Note: The Memory care unit at this facility remodeled the kitchen area. The remodeled area includes a household Maytag dish machine with NSF household certification. Equipment shall be washed, rinsed, and sanitized, in the commercial dish machine in the kitchen area, or provide a commercial dish machine in this area.   • Observed Vulcan oven model number VC-4GD-10, and Beverage Air milk cooler was added to the operation.

Observed pasteurized whole eggs were used for fried eggs for residents.
One accurate temperature measuring device was available for use.
Foods were properly cooling under refrigeration.
Observed raw animal foods were properly stored according to minimum cooking temperatures.
  • 6.4/4.5 The following areas were soiled:
  • - the floor under the dish machine
  • - exterior of the pickle bucket
  • - floor drain under the preparation sink
  • - door seal on the walk-in cooler, and the Victory juice unit
  • - hood above the dish machine
  • Clean these areas.
  • 4.4 The convection oven was intermittently working, additionally the other oven on premise was not properly functioning. Repair the ovens to assure food items can be properly cooked within 4 hours, and reheated rapidly within two hours.
  • 6.2 Repair the non-functioning light fixture under the hood unit to provide 50 foot-candles of lighting.
  • 6.4 Repair the damaged wall by the hand sink to provide a cleanable surface.
  • 6.0 Remove the carpet rug from the employee's restroom to provide a cleanable surface.
  • Note: The Memory care unit at this facility remodeled the kitchen area. The remodeled area includes a household Maytag dish machine with NSF household certification. Equipment shall be washed, rinsed, and sanitized, in the commercial dish machine in the kitchen area, or provide a commercial dish machine in this area. Reportedly dishes are returned to the kitchen for cleaning and sanitizing.
  • PROTECTION FROM CONTAMINATION: Observed a can of peaches was dented on the top seam. Packages of foods shall be in good condition to protect the integrity of the contents and to prevent potential contaminants. This food product was removed from the normal stock this morning.

DATE MARKING: Observed turkey salad was date marked 10/5-10/12/10, and ham bone was dated 10/6/-10/13/10, and sliced tomatoes were dated 10/7/10. Ready-to-eat foods shall be dated the day of opening or when removed from the freezer, and shall be used or discarded in seven calendar days. These food items were properly dated or discarded today.
Observed pasteurized whole eggs were used for fried eggs for residents.
R/TEMPERATURE MEASURING DEVICE: Observed 1 one temperature measuring device was 24°F in an ice bath.
Temperature measuring device shall be accurate within +/-2°F to monitor temperatures of foods. This device was calibrated today.
