  • *3.4 Observed chili from the previous day contained a temperature of 44°F after cooling overnight. Foods shall be cooled rapidly from 135°F-70°F within two hours and from 135°F-41°F within a total of six hours to limit the growth of organisms.
  • R/*3.4 Observed the pepperoni in the walk-in cooler was lacking date marking. Ready-to-eat, potentially hazardous foods (PHFs) shall be dated the day of opening or preparation and used or discarded within 7 days.
  • 6.2 Both light shields or/and end caps were lacking in the deli case. Provide this to prevent contamination of food products.
  • 6.1 Ceiling tiles, frame work for the ceiling tiles in the rear deli area were soiled, or contained chipping paint. Clean or repair these areas.
  • 4.5 Clean the soiled caulk above the three-vat sink.
  • 6.2 Repair the non-functioning light fixture under the hood unit, to provide lighting as specified in OAC 3717.
  • 4.4 Resurface the bare wood surrounding the hole in the counter, to provide a cleanable surface.

  • - Salt cured pork was stored with smoked sausages and ham cubes
  • - Turkey wings and drumsticks were stored with bacon and smoked pork spare ribs
  • - Raw pork shoulder roast was stored with cooked ham
  • - Raw bacon was stored with cooked ham
  • - Chicken and turkey was stored above raw pork and ready-to-eat sauerkraut
  • - Raw sausage was stored above whole beef
  • - Raw pork was stored above sauerkraut
  • Store raw animal foods separate from and below ready-to-eat foods in order to prevent possible cross-contamination.
  • 3.5 Provide ingredient labels on the turkey wings and drumsticks, with the ingredients in descending order.
  • R/6.4 Repair the missing/damaged coving throughout.
  • 6.4 Clean the wall surface below the three vat sink, which is soiled.
  • 6.4 Repair the uncleanable ceiling tile in the meat area, and duct taped outlet covers, to provide a cleanable surface.
  • 6.2 Repair the non-functioning light fixtures throughout as needed to provide lighting as required in OAC 3717.
  • Note: Discussed with PIC the labeling on the smoked pork hooks, cottage butt, Canadian bacon, and turkey wings, and turkey drumsticks were lacking cooking instruction or specified if the product was raw or a ready-to-eat food, all distributed by Falters
  • Products. This inspector asked the PIC to acquire additional information for these products. Also discussed the Lee Brand
  • Prepackaged products which contains ready-to-eat hotdogs, kielbasa and raw bacon packaged together.
  • DAIRY:
  • 6.2 Repair the non-functioning light fixtures in the walk-in cooler, to provide 10 foot-candles of lighting.
  • Discussed light shields in the walk-in cooler with PIC.
  • BAKERY:
  • C/*7.0 Observed two chemical spray bottles were unlabeled. Properly label toxic chemicals with the name of the food to avoid confusion and misuse.
  • 6.4 Clean the floor throughout the fryer area, which contains grease accumulations.
  • R/6.4 Repair/replace missing tile grout and pitted floor surfaces in order to provide a cleanable surface.
  • NOTE: Floor surfaces are listed on a signed compliance agreement with a correction date of 12/2/11.
  • 4.5 Clean the dusty condenser and filters on the refrigerated bakery case and upright freezer unit.
  • 6.1 Provide lacking light shield or end caps on the light fixtures throughout the refrigerated bakery case, to prevent contamination.
  • 6.4 Clean the floor by the oven and proofer unit, floor in the walk-in freezer unit, and the floor surface inside the proofer unit, which are soiled.
  • 4.4 Repair the unsecure window surface on the proofer.
  • 3.2 Label the water spray bottle with the name of the food, to avoid confusion and misuse.
  • 4.4 Repair the damaged door seal on the walk-in freezer door, to provide a cleanable surface.
  • 5.1/6.4 Hot water tank appears to be leaking. Repair this area and clean the floor in this area.
  • C/3.5 Observed the sprinkles and M&M ingredients were lacking on the Fudge Iced Brownies, also observed chocolate, chocolate chip cookies with pink M&M's were labeled Iced brownie cookies. Properly label these items with the ingredients in descending order.

  • 4.5 Clean the fryer legs in order to remove grease build-up.
  • -2 accurate long stem thermometers available for use.
  • -Foods were hot and cold holding at proper temperatures.

  • R/4.4/6.4 Repair the damaged walk-in freezer door frame and floor in order to keep equipment in good repair and provide a cleanable surface.
  • R/4.5 Observed ice build-up in the damaged walk-in freezer. Repair and maintain foods protected from possible contamination.
  • RETAIL:
  • 901:3-4-15 Observed the following baby foods for sale beyond the expiration date:
  • -6 Enfamil October 2010 and February 2011
  • -7 Gerber Grape Juice December 2010

  • Do not offer baby food for retail sale beyond the expiration date. Discard baby food. PIC separated the expired baby foods and agreed to discard.
  • PRODUCE:
  • C/3.5 Observed a package of Fajita Mix lacking a label. Label all packaged foods with name, weight, place of business, and ingredients in descending order.
  • NOTES:
  • -Deli: Observed a small container of Half and Half in the walk-in cooler with an expiration date Jan 11, 2011. Discard food as necessary.
  • -Bakery: Discussed cleaning donut racks daily.
  • -Meat: Discussed contacting the Manufacturer of Schooner Shrimp Crunchies in order to determine if it is a ready-to-eat or raw food. Label indicated heat and serve.

  • 50°F, and the macaroni salad was holding at 55°F. Refrigeration units and foods shall be maintained at 41°F and below to limit the growth of organisms.
  • 6.4/4.5/4.4 The following areas were soiled or damaged:
  • - safety release handle in the walk-in cooler is damaged
  • - hood surface is chipping
  • - ceiling surfaces are stained, soiled or uncleanable
  • Clean or repair these areas.
  • DAIRY:
  • 6.2 Repair the non-functioning light fixtures in the dairy walk-in cooler, to provide 10 foot-candles of lighting.
  • RESTROOMS:
  • 6.2 Repair the non-functioning light fixtures in the women's restroom.
  • 6.4 Continue to make necessary repairs to the walls, lacking base cove, caulk, and floor tiles in the men's restroom to provide cleanable surfaces.

Store raw animal foods separate from or not above ready-to-eat foods to prevent cross-contamination.
  • R/6.4 Repair the lacking base coving in the meat area, to provide a cleanable surface.
  • R/6.2 Repair the non-functioning light fixture in the walk-in cooler, and walk-in freezer, to provide 50 and 10 foot-candles of lighting respectively.

  • R/6.4 Clean or repair the ceiling tiles and ceiling vents throughout, which are dusty or contain chipping paint. Per a compliance agreement dated 5/28/10 with this office correction date for this item was 11/28/10.
  • R/6.4 Repair the pitted floor surface by the bulk food containers, to provide a cleanable surface.

  • - floor surfaces throughout as needed
  • - floor in the walk-in freezer including under the storage racks,
  • - floor/cabinets juncture by the icing tables

  • 5.1 Repair the leaky faucet on the three vat sink. Plumbing shall be maintained in a state of good repair.
  • FROZEN FOODS:

  • R/- The walk-in freezer had broken door frame exposing insulation in the walls
  • - lacking hinge on the walk-in freezer door
  • - damaged condenser covers, and ice accumulation on and in the condenser unit
  • - floor surface in the walk-in freezer is unsecure
  • Equipment shall be maintained in a state of good repair.
  • 4.5 Clean the ice accumulation in the ice cream walk-in freezer, and the rusty floor surface in this unit.

  • R/4.1/9.0 Observed Hamilton Beach Food processor. Reportedly, the food processor is used for onions only. If this appliance is used for potentially hazardous foods or the menu changes, equipment must be replaced with NSF approved or commercial grade equipment.
  • Note: Observed the hand sink hot water in this area was 66°F, until a valve in the back room was turned off. The hot water then quickly reached 100°F. Make necessary repairs in this area.
  • RESTROOMS:
  • R/6.4 Continue to make necessary repairs to the walls in the men's restroom to provide a cleanable surface.
  • RETAIL PRODUCE:
  • R/*3.4 Observed cut melon was holding at 43°F, 45°F, 50°F, 44°F, and cut honeydew was holding at 43°F, 48°F, and 49°F.
  • Maintain potentially hazardous foods at 41°F and below to limit the growth of organisms.
  • MEAT:
  • R/*3.2 Observed the following food items were improperly stored: raw salt pork was stored with hotdogs, and above ready-to-eats ham. Store raw animal foods separate from or not above ready-to-eat foods to prevent cross-contamination.

  • R/6.4 Repair or replace the lacking grout between the floor tiles throughout, to provide cleanable surfaces.
  • R/4.5 Clean or replace soiled or damaged caulk at the backsplash hand sink juncture by the hot holding case.

  • R/6.4/4.4 Floor by the mixer contains pooling water from the proofer. Clean this area, make any needed repairs to the proofer.

  • R/The walk-in freezers had excessive ice accumulation

  • Equipment shall be maintained in a state of good repair.

  • R/4.4 The following areas are damaged:
  • - door seals on the display doors holding Busch beer, Coors beer, Budweiser beer, and both specialty beer display doors.
  • Repair these areas.

  • R/6.1 Eliminate the air gap under the side delivery door to prevent pest entry.
  • Note: A compliance agreement was signed between this office and the above operation, outlining correction date for many of the repeat structural violations.
