- R/- wall and base cove under the hand sink and surrounding the mop sink
- R/- ceiling tiles above the 3-vat sink is damaged
- R/- shelves in the walk-in cooler are rusty
- - plastic door frame surface on the walk-in freezer door is damaged and contains ice accumulation
- R/- ceiling tile by the walk-in cooler is damaged
- - Cres core hot holding case top door seal on the backside of the unit is damaged
- - Delfield unit is leaking into a pan
- - bottom door seal on the Delfield refrigeration is damaged
- - Window surface on the hot holding unit was duct taped
- - repair the damaged wall tiles above the hot holding case
- - damaged ceiling and wall paper surface in the women's restroom
- Repair these areas, to provide cleanable surfaces.
- 4.5 Clean the interior of the steam table unit which contains mineral deposits.
- Note: This office received a transfer form & license from this operation. This inspection will count as the first standard inspection for this operation. Reinspection will be conducted during the week of June 13th, 2011 to assure above violations are corrected and this operation is in compliance prior to license transfer.

- R/6.4 Observed ceiling and wall paper in the women's restroom was damaged, ceiling tiles by the three vat sink and rear door were damaged and uncleanable, the coving surrounding the mop sink was damaged and lacking, damaged or lacking floor grout throughout the operation, and the walk-in cooler back wall was damaged. Repair these areas.
- R/6.2 Provide 20 foot candles of lighting above the three vat sink, currently 13 foot-candles of lighting.
- 4.5 Clean the breading rack, which contains grease accumulation.
- 6.1 Eliminate the air gap under the rear door to prevent pest entry.
- 4.4 Repair the damaged door seal on the walk-in freezer, and the rusty shelves in the walk-in cooler, to provide a cleanable surface.
- 6.4 Clean the ceiling tiles throughout which are stained or soiled.
- 4.4 Repair the hot water dispenser, and the cabinets under the drive thru hand sink which are buckling and uncleanable.
- 4.4 Repair the damaged door seal on the bottom glass door of the Delfield unit.

- R/6.4 Ceiling and wall paper in the women's restroom was damaged, ceiling tiles by the three vat sink and rear door were damaged and uncleanable, and the coving surrounding the mop sink, damaged or lacking floor grout throughout the operation, damaged wall tile by the single-use storage shelves, and the walk-in cooler wall were damaged. Repair this area.
- R/6.2 Provide 20 foot candles of lighting above the three vat sink, currently 10 foot-candles of lighting.
- R/4.5/3.2/6.4 Observed ice accumulation in the walk-in freezer on the condenser, food shelves, food products, and walls. Defrost and repair this area. Discard any contaminated food products.
- 4.5 Clean the steam table wells, breading rack, which contains grease accumulation or are soiled.
- 5.1 Spray hose has a towel wrapped around it, remove the towel and make any necessary repairs.
- 6.1 Eliminate the air gap under the rear door to prevent pest entry.
- 4.4 Repair the damaged door seal on the walk-in freezer, and the rusty shelves in the walk-in cooler, to facilitate with temperature maintenance.
- 6.4 Clean the wall surfaces in the breading area, which contain grease accumulation.
