  • R/4.1/9.0This operation has eliminated many of the departments previously in this store. The retail areas includes 3 upright dairy cases used to store dairy items, lunch meats, raw eggs, and beverages. This operation Risk Level will change to a Risk Level 1<
  • 25,000 square feet for the 2012-2013 licensing year. Operation hours have changed to 8am- 10pm. Provide this office with an update layout of the operation including, the location and make and model numbers of the equipment, and surface finishes.
  • 901:3-4-15 Observed two containers of Enfamil Gentlease formula which expired on 12/1/11. Check and rotate products regularly. Removed expired products from the sales floor. These items were removed from the sales floor today.
  • *7.1/7.2 Observed on the sales floor and throughout the storage area toxic chemicals were stored with and above equipment.
  • Toxic chemicals shall be stored in a separate designated area or not above food and equipment to prevent contamination.
  • Relocate these chemicals to prevent contamination.
  • *3.1/6.3 Observed dented cans of Campbell's chicken noodle soup, Progresso beef pot roast soup, and Van de Camp Baked beans
  • (2 varieties) on the retail sales shelf. Packages of food shall be in good condition to protect the integrity of the contents and to prevent potential contaminants. Relocate these canned goods from the sales floor.
  • 4.4 The upright refrigeration case with the hot dogs contained and air temperature of 42-43°F. Adjust this unit to assure 41°F and below is maintained.
  • *3.2 Observed raw turkey bacon was stored above Dole fruit cups. Different types of raw animals food shall be stored separate from or not above ready-to-eat foods to prevent cross contamination.
  • Note: Observed several containers of Dairymen's milk contained a sell-by, use-by date of 11/26, and 12/4. Check and rotate stock regularly.

The following violation were observed during this inspection:
  • 901:3-4-15 Observed 6 boxes of Baby Mum Mum rice rusks with an expiration date of 6/11/11. Check and rotate products regularly. Removed expired products from the sales floor. These items were removed from the sales floor today.
  • 4.4 Repair the two damaged door seals on the first upright dairy case to facilitate with temperature maintenance and to provide a cleanable surface.

  • This operation will reportedly eliminate many of the departments of this retail store by April 1, 2011. During this inspection it was observed the produce and meat departments have been eliminated. Reportedly this operation intends eliminate the current retail area to several retail aisles and cooler units. Provide this office with the new layout of the operation, including the make and model numbers of equipment. Currently the operation will stay a risk level 3 operation for the 2011-2012 licensing year. This operation is currently slicing lunch meats and preparing foods for hot holding.
  • DELI DEPARTMENT:
  • *3.4 Observed the chicken pieces were holding at 126°F, the cheese island contained an air temperatures of 43°F-47°F, and the foods in this unit were holding at 45°F-47°F. Refrigeration and foods and be maintained at 41°F and below or 135°F and above to limit the growth of organisms. Discussed with PIC to discard food products above 45°F and above, and to properly reheat the chicken pieces to 165°F and above within two hours.
  • 4.5 Clean the bottom of the Lang unit, which is soiled.
  • 6.2 Repair the non-functioning light fixtures under the hood unit.
  • FROZEN DEPARTMENT:
  • 4.4 Observed food items in the frozen case with the Ore Ida frozen tater tots were thawing this unit, was blowing warm air ,and contained inconsistent air temperatures. Maintain foods in a frozen state. Repair or relocate foods from this unit.
  • 4.5 Observed ice accumulation in the large walk-in freezer. Clean this area.
  • MEAT DEPARTMENT
  • R/*3.4 Raw country ham slice was stored above sauerkraut in the retail case. Different types of raw animal food shall be stored separate from other raw animal foods and separate from ready-to-eat foods to prevent possible cross contamination.
  • R/6.4 Repair the damaged/uncleanable walls in the meat department, also repair the holes on the wall surfaces by the meat grinder, and the wall surface by the retail meat case which is damaged and lacking base cove.
  • SANDWICH KIOSK:
  • 4.4 Repair the rusty shelf in the Delfield unit to provide a cleanable surface.
  • DAIRY DEPARTMENT:
  • *3.4 Observed the dairy case with the iced tea and Blue Bonnet butter contained an air temperature of 45°F, and the foods in this unit such as but not limited to Blue Bonnet butter and American cheese were holding at 46°F. Refrigeration and foods and be maintained at 41°F and below to limit the growth of organisms.
  • 6.4 Repair the damaged floor tiles in the large walk-in cooler, to provide a cleanable surface.
  • 6.2 Repair the lights in the walk-in cooler as needed, to provide lighting as specified in OAC 3717.
  • 3.2 Store food six inches off the floor to facilitate cleaning and to prevent contamination.
  • RETAIL AREA:
  • *901: 3-4 15 Observed the following baby food items were expired:
  • - 5 containers of Mixed fruit juice 4-pack dated 2/10/11, 1/13/11
  • - 2 packages of Gerber Sweet Potatoes baby food was dated 12/8/10
  • - 1 package of Gerber Squash was dated 1/8/11
  • - 1 package of Gerber White Grape juice was dated 11/30/10
  • Remove expired goods from the sales floor. Check and rotate stock regularly.
  • 4.4 Repair the damaged door seal on the Faygo unit, to provide a cleanable surface.
  • RESTROOMS:
  • 6.2 Provide a hand washing sign in the men's employee restroom, to inform employees to wash hands.
  • 6.4 Repair the damaged base cove in the men's employee restroom, to provide a cleanable surface.
  • BAKERY DEPARTMENT: Note: The dish machine was not checked during this inspection. This operation as a three vat sink with sanitizer and test kit for sanitizing equipment.
  • R/4.5/4.4 Observed ice accumulation on the ceiling, walls, and floor in the walk-in freezer. Continue to clean and repair this area.
  • R/6.4 Clean the soiled ceiling tiles in the bakery area. Ceiling tiles must be replaced with a cleanable tile.
  • R/6.4/4.4 Remove the unused, unnecessary proofer from the bakery area, the seal on this unit is also damaged. Repair this area.

  • *3.4 Observed the deli case with the deli salads and sliced lunchmeats contained an ambient temperature of 41-42°F, and foods in this unit contained temperatures of 41-44°F. Refrigeration and foods and be maintained at 41°F and below to limit the growth of organisms.
  • Discussed when the non commercial microwave does not function it must be replaced with a commercial microwave.
  • BAKERY DEPARTMENT:
  • R/4.5/3.2/4.4 Observed ice accumulation on the condenser, door, air curtains, and floor in the walk-in freezer. Continue to clean and repair this area.
  • 4.4 Repair the damaged condenser cover in the walk-in freezer.
  • 6.4 Repair or clean the soiled or damaged ceiling tiles in the bakery area. Ceiling tiles must be replaced with a cleanable tile.
  • 4.5 Clean the dusty hood filters in the hood unit above the icing table.
  • 6.4 Remove the unused, unnecessary proofer from the bakery area.
  • 6.4 Clean the wall surface by the gray storage cabinet, which is dusty.
  • 4.5 Provide sprinkle ingredients for the cupcakes on the ingredient labels.
  • SANDWICH KIOSK:
  • No violations were observed in this area.
  • FROZEN DEPARTMENT:
  • 4.5 Clean or defrost the ice accumulation in the large walk-in freezer, and in the ice cream display case, also repair the damaged door seal on this unit.
  • MEAT DEPARTMENT
  • C/*3.2 Observed raw salt pork was stored with fully cooked ham. Different types of raw animal food shall be stored separate from other raw animal foods and separate from ready-to-eat foods to prevent possible cross contamination.
  • 6.4 Repair the damaged/uncleanable walls in the meat department, also repair the holes on the wall surfaces by the meat grinder.
  • PRODUCE DEPARTMENT
  • 6.2 Repair the non-functioning light fixtures throughout the produce back room.
  • 6.4 Provide a drain cover on the floor drain in the produce back room.
  • *4.5/4.6 Observed a soiled knife in the knife box. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch.
  • *4.4/4.5/4.6 Observed Glass cleaner was attached the sanitizer dispenser on the two compartment sink. Food contact surfaces shall be washed, rinsed, and sanitized, and shall be clean to sight and touch. This bottle was removed today.
  • *6.2/5.1 No paper towels were available at the hand sink and the pedal for the hot water was not properly functioning. Provide paper towels and repair the pedal on the sink for the hot water, to facilitate proper hand washing.
  • *4.4 QAC sanitizer in the spray bottle and wiping cloth bucket contained greater than 400 ppm. Maintain QAC sanitizer at 150-400 ppm per manufacturers specifications.
  • 3.2 Relocate produce rack next to the hand sink to prevent potential contamination of produce.
  • DAIRY DEPARTMENT:
  • 6.4 Clean the ceiling surface in the large dairy walk-in cooler, which is soiled.
  • RETAIL AREA:
  • *7.2 Observed Glass cleaner stored with ketchup, and laundry soap stored with containers in the dollar discount area, and Plink garbage disposal cleaner stored by the canned tomatoes. Store toxic chemicals in a separate designated are from food and equipment to prevent contamination.
  • 6.2 Repair the non-functioning light fixtures in the beer cases, to provide lighting as specified in OAC 3717.
  • C/3.2 Store the tortilla chips six inches off the floor to facilitate cleaning and to prevent contamination.
  • 6.4 Repair the damaged wall surfaces in the mop sink area to provide a cleanable surface.
