- - Duct taped rack in the walk-in cooler
- - grout between wall tiles in both restrooms.
- Correct these areas.
- 3.2 Label the seasoning shakers and bulk foods containers in the waitress area and by the pizza oven to avoid confusion and misuse.
- 4.4 Observed rusty shelves storing pizza pans and bulk foods. Repair these areas to provide a cleanable surface.
- 4.8 Invert the storage of utensils in the container by the reach-in cooler to prevent contamination of the food contact surface.
- 6.1 Provide shatter proof bulbs or end caps on the light fixtures above the salad and buffet bars, to prevent contamination.

- R/*3.4 Three bean salad in the walk-in cooler was dated 7/28/11. Food shall be dated the day of opening and shall be used or discarded in seven calendar days.
- *6.2/2.4 No hand sink was available for use. Reportedly hand sink will be replaced today. Currently, the preparation sink is being used for hand washing. Provide a hand washing sink, and wash and sanitize the preparation sink before used for food preparation.
- *7.1 Observe a blue chemical spray bottle on the equipment rack. Store toxic chemicals in a separate designated area from food and equipment to prevent contamination.
- *3.4 Hard boiled eggs in the walk-in cooler were holding at 45-46°F. Maintain foods at 41°F and below to limit the growth of organisms . This product was discarded today.
- *2.4 Observed an open beverage container on the dough preparation table from the previous evening. Employees shall consume beverage from an open container in a separate designated area, or from a closed container throughout the operation.
