--Cooked bacon bits observed at room temperature. Provide Pennsylvania Department of Agriculture approval showing this product is shelf stable. Supply the required information within 7 days. In the interim maintain bacon bits at either a hot holding temperature of 135 degrees F or above, or at cold holding temperature of 41 degrees F or below. Corrected on site.

--Observed an ice build-up on the floor of the walk-in freezer. Have the ice removed within 48 hours. Have the unit served and any necessary structural repairs completed within three weeks.

--Ms. Brandi Snyder has completed a recent ServSafe Certified Food Manager Coursework and is awaiting her certificate. Upon receipt Ms. Snyder must apply for reciprocity.
