FACILITY IS A PARK DISTRICT GOLF COURSE. UuPON ARRIVAL THE POSTED SIGN STATED LAST DAY WAS NOVEMBER6, 2011.  THE STAND EQUIPMENT WAS BEING WINTERIZED AND ONLY PREPACKAGED ITEMS - CANDY, CHIPS, SODA ETC. WERE LEFT AND THESE WERE VERY FEW.
NO INSPECTION WAS PERFORMED.

  • Plumbing installed and maintained
    Leak under 3 bay at middle waste pipe around release bar area leaving standing water on the floor. Repair leak.
    Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.
  • Critical: Toxic items labeled and used properly
    Spray bottle of chlorine for sanitizing at a toxic level ( test strip bleached out).
    Chemicals used for sanitizing, shall not have concentrations higher than the maximum permitted under 21 CFR 178.1010.
    *bottle was remade to provide 200 ppm of sanitizing solution*
