Establishment continues to utilize temporary space until water line is installed at other location (not completed today due to rain).  Expected completion 6/19/14.

Restroom agreement was provided.  Variance for permanent hookup to water and electricity was provided.  Screen for window has not been completed.  Worker has agreed t o utilize screened opening at door until screen is installed on window.  Any windows detected open during inspections will be noted as a violation if screen is not present.

Sanitizer - chlorine

Ambient temperature of mechanical refrigeration: 41 degrees F.

Hot water heater has been installed and is in full operation.

Note: Discussed contacting the fire marshal to ensure that the proper equipment and items are in place for fire suppression.

Discussed that the location of the current AirStream Mobile unit will only be there until such time that the water trench and RPZ is provided as per Illinois American Water.

Provide this department with the restroom agreement as discussed in regards to the utilization of a restroom by employees.

The refrigeration unit was not plugged in for a long enough time, however, the temperature has been steadily decreasing.

Chlorine bleach is the sanitizer used in the mobile unit.

The final inspection may possibly be scheduled for tomorrow afternoon pending the placement of the water heater and sewage disposal agreement as well as maintaining records and logs of the portable station pumping or methods for proper disposal.

Provide this office with any information as to known events or additional locations at which the operator shall be preparing and serving food during operational times.

The mobile unit will not be licensed today as there is no hot water and the license still needs to be paid for the mobile unit. A temporary license will be required as 'warm water' will be brought in for the hand washing and three compartment sink, this was applied for and will be valid for 1-3 days. Contact this department to inform us when all items have been corrected.

    There is no drain board for the three compartment sink.
    It was discussed that the sliding area near the door would be used as the drying area.
    Sinks and drain boards shall be self-draining.
    Drain boards and easily movable dish tables shall be located so as not to interfere with the proper use of the dish washing facilities.

    There is no water heater available in the mobile unit.
    A water heater is required for the mobile unit.
    Each compartment of the three compartment sink shall be supplied with hot and cold potable running water.
    Lavatories shall be provided with hot and cold water tempered by means of a mixing valve or combination faucet.

    Provide a written agreement with the Simmons or other waste management company to ensure that the waste is properly and routinely disposed.
    Water-carried sewage shall be disposed of by means of a public sewage system or an approved sewage disposal system which is constructed and operated in conformance with applicable State and local laws, ordinances and regulations.

    Provide screening material to all windows that will be open for circulation. Utilize the screen door to order and open/close the window as needed for delivery of the crepe.
    Screens for windows, doors, skylights, transoms, intake and exhaust air ducts, and other openings to the outside shall be tight fitting and free of breaks.

I can't say the food was good or bad because there was not enough of it to review. A mediocre crepe with 1 slice of deli ham and a little cheese. The only appropriate thing was the name of the establishment as in "wholly crepe did I just pay $5.00 for this 50 cent meal?"
