    Live roach observed in same area as routine inspection (between food preparation cooler and 2 door True cooler.  Worker provided invoice with date of last professional pest control treatment for German roaches.  Observed sticky pads with dead roaches in various places.  Area of concern is very warm and there is a drain nearby for the ice machine with moisure on the floor. Continue to provide weekly pest control and provide written evidence of appropriate treatment.  Recheck on or after 4/28/14.

    Observed a case of eggs on floor in walk in cooler.  Observed cases of shrimp on floor in walk in freezer.
    Containers of food shall be stored a minimum of six inches above the floor or on easily moveable dollies, racks or pallets to protect the food from splash and other contamination, and to permit easy cleaning of the storage area.  Place food product on shelving.

Note: The owner has been working on sealing any holes, cracks and or crevices. Continue to look for holes, cracks and crevices and seal them. Maintain them to be sealed to prevent hiding places and breeding locations for pests.


You are required to maintain all pest control invoices and be able to provide them at the next inspection.

    _Corrected: The population of pests has decreased dramatically. There were only a few live cockroaches seen around the booster heater at the dish machine area. Owner stated he will have his electrician look and see if they even need the booster heater. It appears it is not in operation. If they are not using it the owner stated he will have it romoved. Reviewed the pest control invoice from Alberts Termite & Pest Control. Treatment was done on Monday December 2, 2013. Since there were only a few cockroaches observed and the establishment is actively combating the issue a recheck will be done at the next routine inspection. Continue to have routine pest treatment.
    Effective measures to minimize the presence of rodents and flies, roaches, and other insects on the premises shall be utilized.
12/6/20133rd Recheck
Notes: The base board next to the hand sink near the grill is not sealed to the wall. The pipe going into the wall is not sealed. There are small holes in the wall above where the rice is stored. The hole where the line that goes into the wall from the booster heater is not sealed. The plug in next to the booster heater is not sealed. Seal these areas to help eliminated the possibly of the cockroaches getting behind the walls. This will be rechecked on or after 12/6/13.

The pest control operate will be contacted to discussed any possible areas that are possibly being missed.
  • Critical: Presence of insects/rodents. Animals prohibited
    Reviewed the pest control invoice for Alberts Termite & Pest Control. Last treatment was Monday November 25, 2013. Treatment used was Demand cs (.03) 2pts. also put in place glue monitoring boards. Spoke to the manager who stated they are having the pest control come every week to treat until the problem is no more. At this time, cockroaches are still being seen in the following locations: seen on the booster heater(possibly breading in the booster heater. Ask the manager if they even use that anymore since they use chemicals for the dish machine. He stated he was not sure. Recommended removing it if it is not being used), noticed live cockroaches next to the grill prep cooler near the hand sink(front right wheel) and around the ice machine and Pepsi sliding door cooler. It seems the number of live cockroaches being seen at this time from previous times is decreasing.  A charged recheck will be conducted on or after 12/06/2013
11/26/20132nd Recheck
Violation number 35 comment continued:

Spoke to the manager on duty who said he has been cleaning and also is using a pest spray in the water heater room to combated the cockroaches. Manager also said that the pest control operator will treat again on Monday. Manager was advised not to spray on his own and consult the pests control operator for what approved sprays he is allowed to use. Make sure they sprays are restaurant safe.
Spoke to an operator for Alberts pest control via phone who stated they are using liquid spray, gel, and monitoring traps. They confirmed they will be coming Monday.  A charged recheck will be conducted on or after 11/25/13.|
  • Critical: Presence of insects/rodents. Animals prohibited
    Still observing cockroaches in the following areas: in the hot water heater room, around the water softener unit by the door to the water heater room, on the floor under the chemicals of the dish machine and a joint where a power cord comes from the wall to the booster heater fro the dish machine under the right side counter top. See page 2 under comments for complete details.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    The wall behind the dish machine has various locations where there are gaps. They are where the power cord comes out from the wall to the booster heater, a plug-in box and along behind the dish machine where the RFP board turns to tile.
    Seal these locations to help eliminate any hiding spots for the cock roaches.
11/15/2013Recheck
  • Critical: Presence of insects/rodents. Animals prohibited
    Observed baby roaches under the prep table, in the furnace room and in the mop room.
    Establishment is having routine pest control from Alberts Termite & Pest Control. See attached picture. Observed Gel in the furnace room and in the mop room.
    Manager stated they will be having Alberts come once a week until problem is eliminated.  
    Due the pest control operator treating today and the possibility that the treatment taken effect on the newly hatching roaches a courteous recheck will be on or after 11/15/13 and at that time it will be determined if more rechecks will be needed. Maintain all pest control invoices.
11/11/20132nd Recheck
This is the first recheck inspection of the establishment. I spoke to Ricardo in regards to the establishment and the items that need corrected. Alberts Termite and Pest Control (309-347-3665) was called to provide pest control management to the establishment, a picture of the invoice is attached. The pest control operator (PCO) provided Demand CS 0.06% 3pt, Maxforce Gel 4 grams, FMC-CB SO 3oz, into cracks/crevices, baseboards, spot treatments, and bait placements in the kitchen/dining areas and storage areas, bathrooms, hallways and outside entries.

During the recheck inspection, it was noted that there was improved cleanliness, walls have been repaired at areas where pests were found. Walls have also been repaired near the tortilla chip area. In the walk in coolers, foods were labeled with dates and times of preparation/cooling. There are also shallower pans being utilized to cool/store foods. Hot foods were found to be at or above 140F, cold foods were found to be at or below 41F.

A recheck will be conducted on Monday, 11/11/13,  afternoon to ensure that pest control management is continued, as well as to determine if the door/entrance has been sufficiently repaired. This will incur a recheck fee.

Continue to clean and sanitize all areas and continue with the pest control operations.
  • Critical: Presence of insects/rodents. Animals prohibited
    Juvenile cockroaches were visible around the mop sink area.
    This is a continued violation. Ricardo called the pest control operator to have them come in and spray again. I did discuss the importance of ensuring that continued spraying is done, but besides the application of pesticides, it is important that cleaning be continued, especially under equipment and the floors in the mop sink and water heater room.
    A recheck will be done on Monday for this and will incur a recheck fee.
    During the inspection, Ricardo did call and schedule an appointment for Alberts to come in to the establishment and provide continued/additional pest control spraying on Monday, November 11.
  • Critical: Outer openings protected from insects, rodent proof
    The back door is partially fixed. It appears that the door had been beaten in to correct the gap at the bottom, however, there still appears to be a small area where pests may enter.
    According to employees, the owner is discussing the replacement of the door with the owner of the building, and an agreement has not been reached as to when this will be repaired or replaced. I did instruct the employees to inform the owner that this needs to be corrected. A recheck will be done on Monday to ensure this is corrected. Additional recheck fees will be incurred.
11/8/20131st Recheck
There will be a recheck on or after 11/8/13 to ensure that critical violations have been corrected, as well as a plan is made and in place in order to continue operations. Any and all recheck fees may be associated with the rechecks.

The old pest control was Griffith, according to Jose, he has contacted Alberts Pest Control (309) 685-1464/(309)347-3665 and spoke to Dave Alberts who would be at the establishment 11/5/13 @ 9:15AM for treatment and to serve as new pest control operator.  It is required that a comprehensive pest control contract be maintained with any company that the owner chooses, and must be in place prior to our recheck.

Discussed with the manager on duty and line cook the locations where major repairs need to be made (door, wall behind Aquafina), flooring of refrigerator, general coving and sealing.

HACCP: Proper cooling of potentially hazardous foods. Labeling of toxic substances including cleaners. Proper cooking temperatures. Proper holding temperatures of potentially hazardous foods. Proper hand washing. Protecting foods including ice at all times.
  • Original container, properly labeled, consumer advisory
    Cooling cheese sauce and cooling pork in the walk in cooler was not labeled with a time.
    Provide a date and time of cooling foods.
    Corrected discussed date marking with employees and provided cooling HACCP sheets.
    Label all cooling potentially hazardous food with the date and time of preparation.
  • Food protection during storage, prepartion, display, service, transportation
    1) The ice in the soda ice bins near the bar is not covered. 2) Bags of rice was found stored on the floor in the storage closet.
    Provide a cover over the ice while not in use.
    Corrected by having an employee provide a cover over the ice.
    Food shall be protected from the potential for contamination at all times, including while being stored, prepared, displayed, served, or transported.
  • In-use food dispensing utensils properly stored.
    A strainer scoop was being stored with the handle immersed in the lettuce in water, located in the walk in cooler.
    Corrected by education and the employee removed the strainer scoop.
    Between uses during service, dispensing and incidental utensils shall be stored: in the food with handle extended out of the food, in running potable water dipper wells or clean and dry.
  • Non-food contact surfaces designed, constructed, maintained, installed, located.
    1) The wooden storage rack in the walk in cooler is molded and is absorbent. 2) There was card board between the chip bins that is absorb ant.
    Provide non-food contact surfaces to be smooth, easily cleanable and non-absorbent.
    Multi-use equipment and utensils shall be made and repaired with safe, non-toxic materials, including finishing materials that is corrosion resistant, non-absorbent, smooth, easily cleanable, and durable under conditions of normal use.
  • Food contact surfaces of equipment and utensils clean
    The following food contact surfaces were soiled: the industrial can opener blade, guard of the meat grinder, interior of the microwave, interior of APWyott tortilla chip warmer, shelving in the walk in cooler, shelving in the walk in freezer and the ice baffle of the ice maker.
    Clean and sanitize all food contact surfaces routinely and after use.
    Kitchenware and food-contact surfaces of equipment shall be washed, rinsed, and sanitized after each use and following any interruption of operations during which time contamination may have occurred.
  • Non-food contact surfaces clean
    The following non food contact surfaces are soiled: 1) dish/equipment carts, 2) undersides of the prep and storage tables, 3) base of the blenders, 4) vitamix blender base, 5) handles of the bean bin, 6) handles of the tortilla bins.
    Clean and sanitize routinely and maintain clean.
    Non-food contact surfaces of equipment shall be cleaned as often as is necessary to keep the equipment free of accumulation of dust, dirt, food particles, and other debris.
  • Single service items properly stored, handled, dispensed
    There were various single service articles stored in the outside shed. The shed does not have a door or appropriate protection.
    Single-service articles shall be stored at least six inches above the floor in closed cartons or containers.
  • Plumbing installed and maintained
    1) The hand wash sink by the three compartment sink is leaking. 2) The drainage pipe for the hand wash sink by the cook line is not completely connected/sealed to the sink.
    Repair the leaks and the plumbing.
    Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.

    Live roaches were observed in the buckling wall behind the Pepsi cooler.
    Discussed with the owner who stated he has pest control coming tomorrow at 9am to treat the building.
    Effective measures to minimize the presence of rodents and flies, roaches, and other insects on the premises shall be utilized.
    A rechek will be conducted on or after 11/8/13.
  • Critical: Outer openings protected from insects, rodent proof
    There is a small gap at the bottom of the back door.
    Provide a weather strip or some other fix to seal the gap.
    Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight fitting self-closing doors, closed windows, screening, controlled air currents or other effective means.
    A recheck will be conducted on or after 11/8/13.
  • Floors properly constructed, clean, drained
    1) The floor behind the Pepsi cooler is soiled. 2) The floor in the walk in cooler is no longer sealed and is buckling up causing cracks in the seams. 3) There are various large cracks of the floor in the kitchen. 4) Coving is pulling away from the walls in various areas.
    Routinely clean and maintain clean floors. Repair the floor of the walk in cooler so that it eliminates the cracks/seams. Repair floors and coving.
    Floors, floor coverings, mats, and duckboard shall be maintained clean. Floors and floor coverings of all food preparation, food storage, and utensil-washing areas, and the floors of all walk-in refrigerating units, dressing rooms, locker rooms, toilet rooms and vestibules shall be constructed of smooth, easily cleanable, durable material. The junctures between walls and floors shall be coved and sealed for floors where water-flush cleaning methods are used. The juncture between walls and floors shall not present an open seam of more 1/32 inch.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    The walls in the walk in cooler are soiled. The wall at the soda fountain are soiled. Ceiling vents throughout are soiled. Fan covers in the walk in cooler and freezer are soiled. The wall behind the tortilla chips are in disrepair. The wall behind the aquafina refrigerator is in disrepair.
    Walls, ceilings, and attached equipment and decorative material shall be maintained clean.
    Walls and ceilings, including doors, windows, and similar enclosures shall be maintained in good repair.
  • Premises maintained fee of litter, unnecessary articles.  Cleaning and maintaining equipment properly stored, kitchen restricted to authorized personnel
    The exterior exhaust vent is leaking into a bucket.  Mops and mop heads were found stored outside the back door.
    Provide proper cleaning and disposal of grease from exhaust.
    Maintenance and cleaning tools such as brooms, mops, vacuum cleaners and similar equipment shall be maintained and stored in a manner that does not contaminate food, utensils, equipment, or linens.
    The premises shall be maintained in such condition as to prevent the harborage or feeding of insects or rodents.
