    Single service napkins were found stored on floor of office.
    Single-service articles shall be stored at least six inches above the floor in closed cartons or containers.

    Hand sink located by three compartment sink was found to be disconnected from drain.  -Corrected by repairing.
    Water-carried sewage shall be disposed of by means of a public sewage system or an approved sewage disposal system which is constructed and operated in conformance with applicable State and local laws, ordinances and regulations.

    Found a case of chips stored on the floor at the bar. Foods must be stored 6" off the floor
  • Non-food contact surfaces designed, constructed, maintained, installed, located.
    Non food contact surfaces in disrepair: Found duct tape on the lid to the ice machine bin, found duct tape on cooler lid handle in music bar area, handle missing to the white chest cooler by the dish area. The make cooler on the cook line has water accumulating inside, check the condensate drip line as it may be clogged. Multi-use equipment and utensils shall be made and repaired with safe, non-toxic materials, including finishing materials that is corrosion resistant, non-absorbent, smooth, easily cleanable, and durable under conditions of normal use.
  • Plumbing installed and maintained
    Found women's restroom center stall toilet out of order. Fix toilet. Remove lime accumulation from fixtures in facility. Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.

    Found several of the paper towel dispensers in the facility broken. Fix dispensers. Clean towels need to be protected from drips off of wet hand.
    Lavatories, soap dispensers, hand drying devices and all related fixtures shall be maintained in good repair.

    Found dumpster lid open. Corrected by speaking to the employees. Waste containers stored outside the establishment such as dumpsters, compactor and compactor systems shall be kept covered when not in actual use.

    Found daylight showing in through the side band entry door. The weather strip is in disrepair. Fix weather strip so that no daylight shows in. Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight fitting self-closing doors, closed windows, screening, controlled air currents or other effective means.
  • Floors properly constructed, clean, drained
    The paint on the floor in front of the handicap womens toilet is peeling off the floor. Floors and floor coverings of all food preparation, food storage, and utensil-washing areas, and the floors of all walk-in refrigerating units, dressing rooms, locker rooms, toilet rooms and vestibules shall be constructed of smooth, easily cleanable, durable material.

Anyone handling food in establishment is required to have food handler training.  Writen informaion on how to obtain this training was provided to this establishment.

    Lid of ice machine broken and held together with duct tape.
    Multi-use equipment and utensils shall be made and repaired with safe, non-toxic materials, including finishing materials that are corrosion resistant, non-absorbent, smooth, easily cleanable, and durable under conditions of normal use.  Remove ductt tape.  Repair or replace food contact surface.

    Lids on bulk food containers on shelf soiled with debris.  Clean and sanitize food contact surfaces.

    Baffles in hood soiled with debris.

    Soda gun and soda gun holder at bar is soiled.  Clean and sanitize all food contact surfaces as often as necessary to remain clean.
