    Bucket of ice for mixed drinks at the bar does not have any form of covering or protection against contamination.  Provide a cover in between uses.
    Food shall be protected from the potential for contamination at all times, including while being stored, prepared, displayed, served, or transported.
  • Critical: Outer openings protected from insects, rodent proof
    Observed front door propped open with no screen door present upon arrival for inspection.  Openings to the outside must be protected from the entrance of pests by way of screens, air curtains, or other barriers.  Corrected:  Front door closed.
    Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight fitting self-closing doors, closed windows, screening, controlled air currents or other effective means.

    Community pop corn baskets on the bar are not covered and are exposed to contaminates. Ice bin is not covered or protected between uses.
    Suggest having a cover over the ice to protect it from contamination and to have a container which the bar tender or whoever is working can dish out baskets for customers.
    Food shall be protected from the potential for contamination at all times, including while being stored, prepared, displayed, served, or transported.
  • Food contact surfaces of equipment and utensils clean
    The racks and interior lip of the pizza oven is soiled.
    Clean and sanitize all food contact surfaces.
    The food-contact surfaces of grills, griddles, and similar cooking devices and the cavities and door seals of microwave ovens shall be cleaned at least once a day.
  • Non-food contact surfaces clean
    The door tracks of the beer coolers are soiled.
    Clean and maintain clean all non-food contact surfaces such as the door tracks.
    Non-food contact surfaces of equipment shall be cleaned as often as is necessary to keep the equipment free of accumulation of dust, dirt, food particles, and other debris.

    This is a recheck of the side door.  The door now has weather stripping on the bottom and is tight-fitting.  Violation corrected.  NOTE: If there is no screen door, the large door must remain closed to prevent pest entry.

HACCP-  Discussed need for larger splash guard, moving of ice and cups, or different aerator on handsink behind bar to prevent splash from handsink to soiling ice and cups.

NOTE:  Recheck will be done on/after 8/25/13 to ensure #35 is corrected and side front door is tight-fitting.

    Cloth towel stored underneath beer glasses in the keg cooler.  All food-contact surfaces must be non-absorbent.  Remove towel.

    No chlorine test strips are provided.  A test kit or other device that accurately measures the parts per million concentration of the sanitizing solution shall be provided and used.  Provide test strips for bleach.
  • Critical: Sanitizing concentration
    No concentration of sanitizer was found in the third compartment of sink.  Kitchenware and food-contact surfaces of equipment shall be washed, rinsed, and sanitized after each use and following any interruption of operations during which time contamination may have occurred.  Provide accurate concentration to sanitize food-contact surfaces.  NOTE: Education given.
  • Food contact surfaces of equipment and utensils clean
    The interiors of the pizza oven, Whirlpool refrigerator and the walkin shelving are soiled.  Maintain all food-contact surfaces clean.  Clean items.
  • Single service items properly stored, handled, dispensed
    Papertowels are stored underneath the mens and womens restroom sinks.  Single-service articles shall not be placed under exposed sewer lines or water lines.  Protect papertowels by not storing them underneath wastewater lines.

    Front side door has large opening at bottom.  Openings to the outside shall be tight fitting and self-closing.  Provide weather stripping for door to prevent entry of pests and rodents.  NOTE: Recheck on/after 8/25/13.

    Fan covers in the walk-in are extremely dusty.  Maintain all walls,ceilings and attached equipment clean.  Clean fan covers.
  • Critical: Toxic items properly stored
    Four bottles of raid were stored on top of the Whirlpool refrigerator with several other chemicals.  All pesticides must be stored below and physically separate from other chemicals.  Provide proper storage for pesticides.  NOTE: Raid was moved to the floor under the mopsink.

    There is a leak under the handsink and at the three compartment sink.  Plumbing shall be sized, installed, and maintained in accordance with applicable provisions of the Illinois State Plumbing Code.  Repair fixtures.
  • Containers covered, adequate number, insect and rodent proof, emptied at proper intervals, clean
    Dumpster lid is open. Keep lid closed at all times to prevent pest harborage.
  • Walls, ceilings, and attached equipment, properly constructed, clean
    Fans in walk-in are soiled.  Walls, ceilings, and attached equipment and decorative material shall be maintained clean.  Clean fans.

    Restrooms are not vented.  Rooms from which obnoxious odors, vapors or fumes originate shall be mechanically vented to the outside.

    Leak exists beneath handsink. Maintain plumbing in good repair. Eliminate leak.
