[5] Source sound condition; no spoilage Observed frozen cheese in the back freezer with mold. All food must be kept free of spoilage and/or contamination. Food items were moved from the freezer and were voluntarily discarded. Corrected on site.
[1] Original container; properly labeled Observed three corndogs in the reach-in freezer without plastic packaging. Items were removed from the freezer and put with the employee food in the back freezer as contact said he wanted them. Ensure all staff and public food is segregated and labeled – corrected on site. Observed old; obviously melted and frozen; ice cream bars in the freezer which were voluntarily discarded – corrected on site.

General Comments that relate to this Inspection All food stored properly and no chemicals stored next to food.No damaged food products sold to public.All use by dates current.Lights shielded as required.Grocery store has a janitorial sink being used as a hand sink in only restroom. A separate handsink in the adjacent room next to bathroom has a handsink which is being used as the janitorial sink. Do post a handwashing only sign at handsink in restroom.

General Comments that relate to this Inspection The following items must be corrected by 12/07/11:1) Replace lightbulb in black reach in freezer with a shatterproof bulb.2) Provide thermometers in both black non-NSF cold holding units.Discussed with operator to store only prepackaged sodas in non NSF reach in reefer and place all thermometers in a conspicous place in all cold holding units. 3) Install a mop sink in back room. Observed plumbing available for mop sink in back storage room. Currently; a mop sink is being used as a handsink and a mop sink in only restroom at facility. Owner Mohammad is to call health inspector when above 3 items have been corrected. Do post permanent health permit on site once received via USPS.Notes:All cold holding units checked OK and recorded < 40 F.Facility has approved garbage service.Restroom observed properly stocked and sanitary.Lighting adequate.Reviewed with operator to store all food 6 inches off ground. Do store food away from all chemicals. Do not store chemicals above food. Practice first in first out method of rotation for food products and observe use by/expiration dates. This grocery store will be selling only prepackaged food items only. A risk category 1 will be assigned to operating health permit. After 90 days from this inspection; a routine inspection will be conducted announced. OK to operate; OK to issue permit.
