General Comments that relate to this Inspection Notes: All remaining items from the Opening Inspection have been addressed.-Holes in the wall by the ice machine were sealed.-Caulking was applied by ice machine; dishwasher and on the cook line.-Floors were cleaned.The Routine Inspection will take place within 90 days. Inspector went over the following items during the Opening Re-inspection:-Do NOT allow employees to eat food at the cook line or while preparing food. -Employees MUST change gloves AND wash hands when returning to work from eating; using the restroom; taking out the garbage; etc. and between tasks such as washing dishes and then preparing food. Wash hands and change gloves frequently.-Ensure food is thawed under running water; in the cooler or as part of the cooking process. Do NOT thaw food at room temperature.--Keep food products at least 6 inches off the floor.-Ensure surfaces and prep sinks are cleaned and sanitized between separate food items such as vegetables and meats.-Ensure employees sick with vomiting or diarrhea stay out of work at least 48 hours after symptoms have stopped.-Ensure items that are cooked and cooled go from 135F to 70F within 2 hours and from 70F to 40F in 4 hours. General Comments that relate to this Inspection Notes: Items that required abatement on 02/05/16 have been corrected. Many items that were given a longer abatement date have been corrected as well.-Y valve was removed from the mop sink.-Facility inquired on the vent in dry storage and it is required for air flow.-Area between 2 comp prep sink was clean-Cover was installed over floor sink drain under 2 comp sink.-Ice machine was clean.-All gaskets were replaced and clean.-Floor was clean behind freezer.-Dust was cleaned from conduit.-Rice cooker was replaced with a new one.-Fan guards were clean in beverage cooler.-CFPM certificates were provided.-Wooden top was replaced with granite on the "To go table"-Chest freezers were replaced with new ones.-Caulking was applied in mens and womens restrooms.-Loose tiles by ice machine had been regrouted.-Walls were smooth and had been repainted.-Shelves in walk in cooler have been replaced.-Floor in walk in cooler was clean.The following items must be corrected by 03/05/16:-Seal holes in the wall to the right of ice machine.-Caulk along floor tiles to the right of machine to seal all gaps.-Caulk behind dishwasher and 3 compartment sink to seal all gaps.-Caulk where metal meets wall along cook line to seal all gaps.-Deep clean all floors to remove grease and all food debris from the floor. This includes under the center island and under the cook line. The floor may need to be scraped or presure washed on a routine basis to keep the floors cleaned.In addition to the CFPM listed above the following individuals have CFPM certificates:Lillian YuServ Safe - 1310368712/14/20Dingguo HuangServ Safe - 1309269312/14/16Bihua HuangServ Safe - 1309269212/14/20Per owner no individuals took the CFPM exam and failed the exam within the past year.

General Comments that relate to this Inspection Ok to issue permit.The following items must be addressed within 30 days (02/05/16):-Remove Y valve at mop sink and directly connect chemical dispenser to faucet or install a quick disconnect. (Corrected on site)-Screen or cap hole in ceiling in dry storage.-Deep clean between prep sinks (2 comp)-Install cover over floor sink under 2 comp prep sink.-Have ice machine professionally cleaned and sanitized and depp clean under and behind unit. Keep receipt for WCHD verification.-Deep clean gaskets to all refrigerators and freezers.-Deep clean behind tall white freezer in corner.-Deep clean floors under equipment that make up center islands.-Clean off all dust buildup on electrical conduits.-Replace rice cooker at far end of cook line due to duct tape and broken electrical cord.-Deep clean fan guards in beverage cooler.-Provide WCHD with current CFPM certificates.The following items must be addressed within 60 days (03/05/16):-Replace wooden top on the "To Go" table.-Replace handle to chest freezer with one that covers all holes from original handle.-Regrout loose tiles along wall near ice machine.-Replace missing tile behind dishwasher.-Replace broken and cracked gaskets to reach in unit for appetizers.-Seal holes in wall to right of ice machine.-Deep clean all floors not previously identified.-Recaulk walls behind dishwasher and 3 compartment sink.-Recaulk where metal meets wall behind cook line.-Recaulk around toilet base and behind sinks in restrooms.-Remove all items not used on a regular bases.-Deep clean all walls and FRP board.The following items must be addressed by July 1st 2016:-Replace rusted shelves in walk in fridge; deep clean floors.-Scrape off old glue on walls and repaint walls where paint has peeled away.
