Facility has not had employee(s) take and fail one of the recognized CFPM exams in the past year.Maria E Gomez- Tello M120214 exp. date: 03/10/17 2nd food protection manager. This seasonal health permit is exempt from the CFPM requirement but has 2 food protection managers between two facities. One at Shopper's Square and the other at Meadowood Mall.No samples of food at time of inspection. All food preparation is done at My Favorite Muffin located adjacent to set up of Hickory Farms.This permit is a seasonal health permit which starts operations at end of October and runs through December 26. All cold holding perishable cheese spreads observed cold holding at 40 F or lower. Do store all boxed food off ground in back storage room. Food is to be stored 6 inches off ground to prevent possible cross contamination. Handsink is located at My Favorite Muffin. No open foods are handled except for samples which are done at permitted facility.Food on display oberved sealed and organized. Dates current.

Time is used as a public health control for perishable cheese. Do time mark the cheese and any other perishable food for the time it starts at room temperature and the time it will be discarded within 4 hours. Operator is using 3 hour max holding time for perishable cheese at room termperature.

080) Thermometer missing in cold holding unit. Ensure all refrigeration and freezer units are stocked with a conspicuously placed thermometer in order to monitor unit temperature.East side freezer bunker is missing a thermometer. Thermocouple reading recorded at 30.5 F by health inspector.

This is a seasonal health permit. All food prep is conducted at My Favorite muffin. Please monitor employees for proper handwashing when prepping food samples. Do cover food cold holding bunkers at night with a lid or heavy cardboard to maintain proper cold holding temperatures of 40 F or lower.

Observed two cold holding bunkers of cheese with recorded cheese temperatures between 49-56 F. Potentially hazardous food found out of temperature. Ensure all cold foods maintain 40 degrees F or less. * 70 pounds of perishable cheese voluntarily discarded.Owner and operators to stock cheese bunker no higher than two containers of cheese from bottom of cold holding unit. Please acquire cold holding thermometers and place in a conspicous spot to monitor temperature of cold holding units. Bunkers are to be covered at end of each busines day with a lid or a protective cover to maintain cold holding temperature.Please monitor the temperature of the product by temping the cheese before using cheese for sampling. A bimetallic stem thermometer calibrated to 32 F in ice and water and properly sanitized before temping cheese is a good monitoring process.

Two cold holding bunkers must be monitored for proper cold holding at 40 F or lower. Owner adjusted thermostat at time of inspection. Please have employees check temperatures throughout the day. Corrective action must be taken immediately if product does not cold hold at 40 F or lower. All cutting of sampled products is done with plastic cutlery. No bare hand contact with ready to eat foods. Hands are wased at My Favorite Muffin. Please review with all staff to wash hands prior to putting on clean gloves. Samples are covered and monitored by staff. Samples are discarded every 2 hours.Back stock is stored in adjacent area in a carpeted area. No perishable foods stored in back storage area. Sales area is secured nighly.
