Observed grease and dirt accumulating of the following areas that need to be cleaned and sanitized to prevent vermin attraction: on the back shelves where the doughnut filling containers are stored; the filling containers themselves; the shelving under the prep table; the walls in the back prep area and the hot water heater next to the 3 compartment sink.Observed extensive ice accdumulation in the front reach-in freezer. Need to defront and clean unit to prevent food contamination.

Discussed with operator to keep all potentially hazardous food cold holding at 40 F or lower or hot holding at 140 F or higher. * Corrected on site.

All food must be stored 6 inches off ground to prevent possible cross-contamination.
