    Observed corner of doorway in back storage room with stud exposed. Observed left side of wall by the back door with sheet rock pulled away from wall.Repair walls and replace base coving to ensure surfaces can be maintained smooth and easily cleanable and to prevent entry of vermin.

    Notes:-hand sinks stocked; observed excellent employee handwashing. One employee stated that handwashing was important to prevent foodborne illness. -gloves are not currently used; discussed future regulations of no bare hand contact with ready to eat food and proper glove use-no cooling or reheating takes place-cold holding good: make up unit all product 36-39F; walk ins all product 37-39F.-hot holding pizzas at 150F or above-date/time marking procedure in place-3 compartment sink and test strips available - 200ppm quat-sanitizer buckets at 500ppm quat. Operator stated the dispenser unit is being fixed. Monitor buckets to maintain concentration between 200-400ppm quat.-food properly stored-discussed first in; first out procedures with no issues noted-chemicals are labeled and properly stored-restroom clean and stocked-dumpster area ok-no pest issues noted at time of inspection - facility has regular pest control -discussed employee health -- health policy posted - Ensure anyone with gastrointestinal symptoms (vomiting / diarrhea) is excluded for 48 hours after symptoms stop.

Tiles must be replaced. This is a violation from last inspection 2/28/11. If tiles are not repaired with given time frame this facility will have re inspection fees applied after the first re inspection. If tiles are not repaired or replaced this facilities health permit to operate will be temporary suspended until repairs have been completed. Floor repair must be completed by 8/24/12 to prevent further enforcement actions. Any questions call inspector @ 328-2434.

    CFPM violation abated on 08/05/10. Mick Ribiero; district manager requested an early reinspection date. Mr. Oscar Parra; assistant manager verified as a full time employee as Little Caesars located on 656 McCarran Blvd.; Sparks. Both Edgar Lomeli; manager and Juan Carlos Lopez; co manager will be taking CFPM course by Tony Pastini on 08/29/10 through 08/30/10.Observed good handwashing at time of reinspection.Facility will have a total of 3 Certified Food Protection Managers designated to this Deli to cover all shifts.

    Reinspection and site visit made to deli and health inspector observed no CFPM on site.Certified Food Protection Manager Certificate for Teresita Lomeli who is superviser for Little Caesars Pizza also shares her time at 3 other pizza locations. Discussed with Mrs. Lomeli that at least one Certified Food Protection Manager who is employed full time (at least 40 hours) must be designated solely to this facility. This was documented on first annual inspection from 01/06/10.This is a repeat violation. You are hereby given notice to abate this violation by 08/27/10. In addition; you will have this CFPM (Certified Food Protection Manager) present during food operating hours or appoint someone in charge.

Ensure all refrigeration and freezer units are stocked with a conspicuously placed thermometer in order to monitor unit temperature.

Mario was not present at facility and also works at Little Caesars at Moano and Northtown Ln. locations.Discussed with operator to have at least one Certified Food Protection Manager assigned to this facility who is employed as a full time employee (at least 40 hours) to this facility by the end of July 1st; 2010.
