Soap was not provided to the kitchen hand sink.
The concentration of sanitizer in the bucket on the three-compartment sink in the kitchen was greater than 200 ppm chlorine.

Chopped beef was stored on top of pork sausage in the walk-in cooler. Moldy fruit was stored on top of a box of wholesome apples in the back storage area behind the meat department. Raw meet was stored above salsa and sour cream in the kitchen cold-top cooler.
An employee in the kitchen was observed cracking raw eggs and then touching clean utensils and tortillas without first washing hands.
Unapproved employee drinks were stored in the meat counter cooler in the meat department and on top of a box of single service items in the back storage area. An open cup of coffee was stored on the meat slicer in the meat department.
A large pan of pork was being reheated in the steam table and was 109�°F-114�°F.
Chicharrones were 71�°F sitting out on top of the steam table.
A pot of cooked tomatoes and jalapenos were 73�°F sitting out in the kitchen. A bucket of fish was 52�°F sitting out in the preparation sink basin. Whole milk cheese was 47�°F in the meat department cold-hold display case. Three slabs of raw pork were 57�°F sitting out on the meat slicer in the meat department. Roasted chilies were 50�°F sitting out on top of the meat debarment cold-hold display case. Sausage was 57�°F hanging from a bar above the meat department preparation table. A block of various meats was 50�°F sitting out on top of the meat department cold-hold display case. A garbage can of raw chicken was 50�°F sitting out in the meat department. Various cheeses were 48�°F-49�°F and hot dogs were 47�°F in the customer cold-hold
