Violation descriptions and comments

Sep 11, 2013

Raw meat was stored above sprouts (open and uncovered) in the reach-in cooler.
Steak is served rare without a consumer advisory.
Cooked meat in a large metal bowl on the prep table partially covered with plastic wrap was 121°F.
Sprouts in the reach-in cooler were 52-57°F. A cooked onion on a shelf was 80°F.
The concentration of sanitizer solution on the dish machine was less than 50ppm chlorine at the dish surface.
The sprayer on the dish machine hung below the flood rim.
Numerous flies were observed in the kitchen.
Unapproved insecticides were in the facility.
An employee was observed filling a container of sauce on one of the customer tables before it was emptied and cleaned. Spring rolls in a cold-top cooler were stored on a shelf outside of a container in a manner that would not protect them from contamination.
The cold-top cooler cutting board was deeply grooved. Food was stored in non-NSF containers throughout the kitchen.
The shelf under the microwave by the fryers was lined with foil.
The exterior of a pan on the stove was heavily coated in burned grease. The lights in the vent hood were soiled with grease and dust. The stove top and the side of the grill were soiled with grease. The exterior of the microwave was soiled with grease. The shelf across from the fryers was soiled with grease and dust.
A spatula by the rice cookers was stored in (73°F) water above 41°F and below 135°F.
Plastic ware were stored with their food contact surface exposed in a container on the kitchen prep table.
Coffee cans were reused to store spices.
The kitchen hand sink by the ice machine was not equipped with a splash guard next to the prep table. The prep sink and back kitchen hand sink were not sealed to the wall.
Floor tiles were chipped and not secure throughout the kitchen. The wall by the warewashing machine was soiled with mold. The floor under the three-compartment sink was grimy. The wall and floor by the hood was soiled with grease. The floor under the prep tables in the kitchen were soiled with food debris.
The back door was left open and the screen door has many gaps.
Food was stored on the way to the customer restrooms.

Raw beef was stored above ready to eat foods in the 2-door standing reach-in cooler.
An employee was observed handling dirty dishes and then clean dishes without first washing their hands.
A cloth towel was stored in the warewashing area handsink.
An employee beverage was stored next to food on the far back area preparation table.
The dish machine failed to produce 50-200 ppm chlorine residual during the final sanitizing rinse.
