Facility closed at time of consultation. Facility to remain closed until a current license is provided.

Attempted hand delivery of food license application to be signed and dated, per clerical. Found facility closed at time of visit. Per clerical, several phone contact attempts have been made and found unsuccessful prior to on-site consultation.

On-site contact attempt for license completion. Phone contacts to facility and owner numbers were unsuccessful. One previous on-site attempt has been made.
Facility was found closed at time of visit.
NOTE: Facility has paid deliquent application fee, but application still needs to be signed and dated for processing.
