  • Do not store or use the boric acid observed under the utility sink area. Label indicated this product was not to be used or stored in or around food prep or food handling areas.

  • Observed several canned food items without labels or half the label is missing. Ensure canned food items have their complete label on the can.
  • Observed several heavily dented cans on shelf. Ensure canned food items are in good condition and are free of dents.
  • Observed bowl of gravy in the walk in cooler that had passed the expiration date on the date label. Ensure all food is used by the date on the date label or discarded.
  • Observed raw shelled eggs stored on shelf above prepared food in the walk in cooler. Ensure all raw animal products are stored below and away from prepared food.
  • Observed multitude of roaches inside kitchen. Live roaches (baby and adult) were observed in ware washing area and surrounding kitchen area. There was a broken piece of tile and once tile was removed, roaches were coming out/going in to hole in the wall. Roaches were also observed in the mop sink room and utility closet. Ensure establishment is treated by a licened pest control and roaches are eliminated.
  • Observed black mold growth on inside panel of ice machine. Ensure the ice machine is clean to sight and touch and cleaned on a routine basis to prevent mold growth.

  • 229.164 (o) (7) (A) consume by date (prepared). Observed containers of prepared foods in walk in cooler without date labels. Ensure all prepared foods have a date label with the USE BY DATE. The USE BY DATE shall not exceed 7 days from the prep day and the prep day will count as day 1.
  • 229.166 (f) (2) (A)HWS>100F. Handsink by exit door did not have running hot water. Employee stated there might be a leak and the valve is turned off.Ensure handsink has continuous running hot water that is at least 100 degrees F.
  • 229.167 (p) (11) (C)insects/pests not minimized. Observed live roaches and egg casings in mop sink closet, chemical storage closet, and bathroom. Last treatment by Orkin was 8-28-12 (no lvie activity noted at that time). Orkin operator stated technician should be out today or tomorrow. Ensure roach activity is eliminated and provide receipt of treatment.
  • 229.165 (r) (3) (D)no sanitization. Observed wares in the sanitizing compartment of the 3 compartment sink without sanitizing soltution. Ensure bleach sanitizing solution is at 50-100 ppm. Obtain testing strips for sanitizer if necessary.
  • Ice machine observed with mold on back panel inside ice machine. Ensure ice machine is routinely cleaned and sanitized to avoid mold accumulation.

  • Observed personal drinks and food stored in freezer and dry storage racks. Ensure personal drinks and food items are stored in a designated area that is separate from food storage that is used for daily operations.
  • 229.164 (r) (1) (A) labeled properly. Observed canned goods in dry storage without labels. Observed pans of donated prepared food in walk in cooler without labels. Ensure all cans that are donated to estbalishment have the label on.
  • 229.164 (v) (5) Donated foods - damaged, etc. Observed heavily dented cans in dry storage area. Ensure all donated canned goods are in good condition and free of dents.
  • 229.164 (o) (7) (A) consume by date (prepared). Observed pans of donated food in walk in cooler without date labels. Ensure all food in walk in cooler that is kept for more than 24 hours has a date label with a "use by" date. The "use by" date should not exceed 7 days from the prep date.
  • 229.166 (f) (2) (A)HWS>100F. Handsink in kitchen that is located near the 3 compartment sink temped at 84 degrees F. Hot water at handsink must be at least 100 degrees F. Ensure hot water at handsink is 100 degrees F.
  • 229.167 (e) (2) no soap. Observed handsink located near 3 compartment sink without soap. Ensure all handsinks are equipped with soap.
  • 229.167 (e) (3) (A) no towels. Observed handsink located near walk in coolers without paper towels.Ensure all handsinks are equipped with paper towels
  • 229.167 (p) (11) (C)insects/pests not minimized. Observed dead roaches throughout establishment, specifically under 3 comaprtment sink and in freezer located in dry storage room. Observed dead flies throughout estbalishment and gnats near ice machine. Ensure all dead pests are eliminated and area is cleaned and sanitized. Use appropriate means to eliminate presence of flies/gnats in kitchen.
  • 229.168 (c) (1) toxics stored. Observed toxic items, such as grease trap cleaner and disinfectant, stored above clean wares and next to dry goods, such as spices. Ensure all toxic items are stored below and away from wares and food items.
  • 229.165 (d) (1) (B)free of breaks, cracks. Observed plates with cracks and chips. Ensure all wares that are used are free of cracks, breaks, or chips. Wares should be smooth and easily cleanable.
  • Ch 13, Art II, section 13-26 Display permit. Food permit was not displayed at time of inspection. Ensure food permit, as well as most recent routine report, is posted in view.
