- Observed improper storage of food items. Observed packaged pizza and doughnuts stored on floor of walk beverage walk-in cooler.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor Store foods at least six inches above floor to prevent contamination..
- Food product labels are not in compliance with standards of identity requirements. Donuts in reach in display case were missing ingredient list.
Packaged food shall comply with standard of identity requirements in 21 C.F.R. 131-169 and 9 C.F.R. 319, and the general requirements in 21 C.F.R. 130. Display ingredient list for reach in donut case in a conspicous place.
- The dispensing tube on bulk milk container was not properly cut as required. Dispensing tubes located on both the flavored coffee machine and chill cream machine were not cut at a diagnol and longer than 1 inch.
The bulk milk container dispensing tube shall be cut on the diagonal leaving no more than one inch protruding from the chilled dispensing head.
- The plumbing system was not properly repaired. Repair/ Replace pipe in walk in beer cooler to elminate ice buildup.
To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code.
- Observed a build-up of dirt and debris. Observed build up of debris located under the coffee and pop machine adjacent to the ATM.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. Clean underneath these areas frequently and thoroughly to deter pests.

- Miscellaneous sources of contamination observed. Found apples on sales floor without protective covering. Food remvoed from sales floor to be wrapped.
Food shall be protected from contamination that is not specified under 3717-1-03.2 (A) to (X).
- The dispensing tube on bulk milk container was not properly cut as required. Tubes found to be about two inches long and cut flat. Operator cut the tubes properly.
The bulk milk container dispensing tube shall be cut on the diagonal leaving no more than one inch protruding from the chilled dispensing head.
