To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.
- Non-food contact surfaces of equipment are dirty. - Observed a build-up of food debris inside microwaves. - Observed a build-up of food debris inside reach-in cooler in sandwich prep area. - Observed a build-up on a few plastic shelf organizers in walk-in cooler.
Nonfood-contact surfaces of equipment shall be kept clean. - Clean and sanitized all noted equipment to remove build-up and prevent contamination of food.
- Observed inadequate number of handwashing sinks. - There is no designated hand washing sink behind deli/sandwich prep counter.
To ensure frequent and proper handwashing, at least one handwashing sink, a number of handwashing sinks necessary for their convenient use by employees in areas specified under paragraph (L) of this rule, and not fewer than the number of handwashing sinks required by the Ohio building code shall be provided.
- Observed unnecessary or nonfunctional items and /or litter on the premises. - Observed unused equipment stored in back room.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter.

- A food packaged in the facility did not contain a label. -Observed packaged sandwiches, coleslaw, antipasta, etc without ingredients on the labels.
Foods shall be labeled as specified in this rule. -Provide ingredients and allergens on labels of food packaged in the establishment.
- Critical: Observed inadequate number of handwashing sinks. There is no dedicated hand washing sink in the deli area.

If the available 3-compartment sink in the deli will be used for hand washing, then utensils may not be washed in the same sink. Facility has an additional 3-compartment sink in the back room that can be dedicated for wash/rinse/sanitize procedures.
- Observed light bulbs or heat lamp that were not properly shielded or coated where required. -Observed uncovered bulbs above the sandwich area.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages.
- Observed unnecessary or nonfunctional items and /or litter on the premises. -Observed unused fryers, displays, etc stored in the back room.
The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. -Remove all unused or unnecessary equipment from the premises.
